FUNDAMENTO.

La RC ha sido un tema controversial en odontologa por
ms de una centuria. Al menos en las ltimas cuatro
dcadas los problemas vinculados con la RC han sido de
inters para los ortodoncistas. La definicin de RC ha cambiado en el ltimo medio siglo pasado desde una posicin
ms retruda y posterior a una posicin anterior y superior.
TIPOS DE ESTUDIOS REVISADOS.
Los autores dirigieron la bsqueda a temas de RC histricos
y contemporneos desde una perspectiva ortodncica. La

Aunque los ortodoncistas gnatlogos contemporneos proponen una ubicacin antero-superior del cndilo a la vez
que los dientes estn en RC (RC=OC) hay poca evidencia
cientfica que soporte este punto de vista27. De hecho, la evidencia sostiene lo contrario. No ha sido demostrado que la
presencia o ausencia de TTM estn relacionados con la
posicin de los cndilos en la fosa glenoidea48-51. Keim52
dijo: la escuela neuromuscular nos dice que existe un rango
aceptable de posiciones (cntricas) .... Si nosotros los clnicos continuamos poniendo nfasis en establecer armona
entre OC y algn concepto mtico de RC, estamos siendo
vctimas de nuestro propio prejuicio.

REGISTROS CNTRICOS:
RELACIN CNTRICA RETRUIDA
Las primeras publicaciones sobre registros de RC estaban
relacionadas con la RC retruda y no con la RC anterosuperior. Ms aun, muchos registros de RC son guiados
manualmente por el dentista (dentista-manipulados)
existiendo diferencias en los hallazgos de registros de RC
manipulados y no manipulados61. Los registros de RC
manipulados (tambin conocidos como registros de RC
con el paciente pasivo) son considerados ms fiables y
menos vlidos75-84. Una investigacin demostr que el
rango promedio de deslizamiento hacia cntrica en re-

gistros repetidos de RC retruda estn 0.30 mm mediolateralmente y 0.27 mm anteroposteriormente85. Parece que los
registros de RC retruda en la odontologa contempornea
tienen sentido slo en la construccin de prtesis completa
cuando no es posible tener referencias interoclusales. Aun
entonces, muchos protesistas usan la RC retruda slo
como una gua para que las dentaduras puedan ser fabricadas a 1 mm por delante de esta posicin.
