Numero OE-2001-73 del 29 de noviembre de 2001, la cual debera leer como sigue:- - - - -

AUTONOMOUS MUNICIPAL GOVERNMENT OF AGUAS BUENAS
ARCHITECTURAL/ENGINEERING SERVICES PROPOSAL
1. GENERAL INFORMATION

According to the public Request for Proposal (RFP) and related documents, the Aguas Buenas
Municipal Government (ABMG) requires the assistance from an Architectural & Engineering
(A/E) firm to prepare a condition assessment and technical evaluation of damages to facilitate
the design of the recovery solution.
The condition assessment will verify the damages sustained by the facility due to Hurricane
Maria and will present the approach to be followed for its "in kind" restoration and
rehabilitation which may require additional specialized studies. C & G

Further technical assessment will identify cause of damage, evaluate risk mitigation, identify
hazard mitigation opportunities, and recommend repair alternatives to the ABMG.
Immediately after receipt of notice to proceed, C&G shall meet with the project Owner to gather
data about the facility, project requirements and schedule.
3. SITE VISIT
C&G shall conduct an ocular inspection of the damaged area. During the site visit the existing
conditions and ancillary components of the municipal facility will be visually inspected. The
condition of the facility and observed damages wil l be documented through notes, sketches, and
photos. The inspection will be limited to ocular observation of accessible areas by experienced
civil engineers. In addition, as part of this task, information provided by the Municipality like
construction drawings and specifications, record drawings, and/or FEMA reports will be reviewed
and evaluated. The information obtained from these documents will be helpful in assessing the
damages sustained by the facility and determining feasible repair options.

A desktop analysis comparing available pre and post Hurricane Maria historical data provided by
the Municipality a nd histo rical imagery published in public data records will be conducted to
associate specific cause of damages caused by Hurricane Maria and identify potential hazard
mitigation opportunities.
4. TOPOGRAPHIC SURVEY
Perfo rm a topographic survey of the project area which will gather, when applicable, re levant
data such as:
• Establish horizonta l and vertical control points within the project areas. Controls will be
adjusted to NAD83 {NA2010) and PRVD02 horizonta l and vertical datums, respectively.
• Gather existing condition survey within the project limits.
• Gather topographic survey data within the project limits in a manner sufficient to
generate a digital terrain model to be graphica lly represented at 0.5 meter contours.
• Gather as-built data of the project site.
• Obtain the site information necessary to establish the amount of damages sustained and
prepare an accurate quantities take-off for a construction cost estimate .

5. EXISTING CONDITION ASSESSMENT REPORT Godinez
Upon completion of the above-mentioned tasks, an existing condition assessment report will be
prepared . Statements addressing the existing condition of the facility will be based on visual
observations made during the site visit for the purpose of providing an overall reconnaissance
level condition assessment of the facility and identify areas requiring repair. Deficiencies
identified from observed damages and/or deterioration will be documented and evaluated. The
assessment is not intended to be a comprehensive evaluation of the quantitative structural
adequacy of the facility but qualitative description of the damages. Data gathered from any
specia lized studies will become integral part of the report which will include the fol lowing
information:
• Summarize the data gathered for the report preparation and provide a description of the
ana lysis and conclusions drawn from the data review and analysis. As part of this section
the FEMA reports documenting e ligible damages will be evaluated and va lidated. If it is
determined that additional data is required for the detailed design of the repairs such
data will be listed in this section of the report for future scoping reference.
• Summarize the damages observed and documented during the ocular inspection for each
infrastructure component of the facility. ·

C&G shall formulate preliminary construction documents to restore and/or rehabilitate the
facilities to pre-existing conditions according to previously est ablished criteria.
C&G shall provide a code compliance opinion of the existing structures and the proposed repair
alternatives. In addition, as feasible, possible hurricane damage mitigation measures and repairs
will be presented. Mitigation measures would be repairs above and beyond the repair "in kind"
option that will provide the structure an enhanced capacity to resist environmental loads caused
by future storms.
Feasible hazard mitigation opportunities will be presented. The benefits and challenges of each
alternative will be evaluated and a preferred repair alternative will be recommended.

6.3.1 - POTENTIAL MITIGATION RENGLON 2 - ROAD LOMA ALEGRE
During this phase C&G shall review the scope of work and design program to schedu le it
according to the time milestones delineated by the Aguas Buenas Municipal Government,
(ABMG). It includes, as needed, the commissioning of topographic, utility survey, and
geological study. Schematic Drawings, with site plans and sections will be submitted to the
ABMG for evaluation and approval that will illustrate the scale and relationship of all project
components and systems, including materials and other appropriate elements. This phase will
include the submission of the H-H study to the DRNA for approval when applicable.
Based on the approval by ABMG of the Schematics Drawings, documents will be submitted for
consultation to the government agencies having jurisdiction over the project's construction
permit. These documents, drawings and preliminary estimate will describe the size and
character of the project as to engineering, architectural, structural, mechanical and electrical
components.

7.2.1 Construction Documents
Based on the approved Preliminary Phase Documents and any further
adjustments in the scope or quality of the Project or in the construction budget
authorized by the ABMG, this phase includes Construction Documents consisting of
Drawings and Technical Specifications setting forth the requirements for the
construction of the Project in detail. It also includes adjustments to previous preliminary
estimates of Construction Costs indicated by changes in requirements or market
conditions. This phase also includes the submission of documents required for the
approval of the construction permit to governmental authorities having jurisdiction.
7.Z.Z Supervision
The Construction Phase commences with the awarding of the Construction Contract and
terminates when the Final Certificate for Payment is issued. During construction our
personnel will make weekly periodic visits to the site to familiarize generally with the
progress and quality of the work and to determine if the work is proceeding in
accordance with the Contract Documents.

On the basis of on-site observations, C&G will advise of defects and deficiencies in the
work and report them to the ABMG. Although C&G will monitor the Contractor's proper
performance of the construction work, C&G shall not be responsible for construction
means, methods, techniques, seq uences of procedures or for safety precautions and
programs related to the work initiated by the Contractor.
Based on such obser.vations at the site and on the Contractor's application for payment,
the recommendation for the Certificates for Payment for such amounts to be paid to the
Contractor, will be prepared for the ABMG revision and payment. Approval of shop-
drawings, samples of materials and equipment, and other Contractor subm issions for
conformance with the design concept of the project and for compliance with the
information given in the Contract Documents, will be implemented, and notified to the
ABMG.

Jose M Castro-Pavia is a professional licensed Landscape Architect & Civil Engineer with vast
experience in site planning and design development. As such, for each project development has
served on interd isciplinary teams working with architects and engineers providing advisory
services during the planning and design process.
The extensive professional education of Jose Castro-Pavia includes two Bachelors' Degrees, one
in Landscape Architecture and another in Civil Engineering. Not the least, professional
education also includes a complete two years curriculum towards a Master Degree in Urban
Planning. The knowledge of these three professions interacted to provide Jose with a v ery
• particular skillset in the practice of Site Planning and Design . With acquired experience, these
skills blended and provided him with a very enjoyable and satisfying professiona l practice.
Recently Jose worked with FEMA as a project specialist of the Hazard Mitigation Grant Program
(HMGP) team within the Transportation Sector. He ga ined valuable knowledge of regulations and
policies applicable to implementing the Public Assistance Alterna t ive Procedures (Section 428).

Jose developed more than 80 Hazard Mitigation reports which included a diversity of subjects
such as waterfront port infrastructure, shoreline stabilization, stormwater management systems,
roadway infrastructure, embankment stabilization, stream restoration, etc. This experience has
nurtured quality skills in the process of a applying forensic engineering to hazard identification
and risk assessment which provide the factual basis of the hazard mitigation plan.
Some of these projects were the, "Paseo Lineal de Puerta de Tierra" which suffered severe coast
erosion, the embankment at the west end of the "Isla Grande" airport runway identified as
"approach end" that services airplanes in their landing approach. The report recommended a
stone revetment which also included hazard mitigation measures.
Architect Jose Godinez and the team at JLGArchitects joins JMCP in these efforts bringing
extensive experience and knowledge in the planning and management of large-scale civil works
among others.
Jose L. Godinez has worked as an architect and eventually a Partner at the Architecture and
Engil")eering Firm Soltero Munoz & Associates (SM+A) where he has worked and lead the design, C & G
Castro &
management, and construction administration of numerous commercial, industrial, medical and Godinez
residential projects.
Most recently, while at SM+A, Jose Godinez led the strategic planning and production of
Construction documents for the Renovation and Expansion of the principal Medical Hospital
Facilities of the Workmen's Compensation Fund Program (CFSE) in the Island; a project that was
conceived and produce in a very aggressive schedule to meet very strict and demanding project
goals.

Prior to the mentioned Medical Facilities Project, Jose Godinez led the coordination and design
of the Civil works of the recently completed Mall of San Juan (a collaboration with engineer Jose
Castro as the lead civil engineer), the design and construction administration of the Home Depot
Building and Site construction Programs in the Island (HD Hatillo, HD Ponce, HD Montehiedra, HD
Los Colobos, & HD Mayaguez), the Costco Warehouse building and site design construction
Programs (Costco Carolina, Bayamon East, Bayamon West, & Caguas), the recent Plaza Carolina
Mall Expansion and Renovation Project, and numerous Walgreens, Best Buy, TJ Max, Office Max,
and Commercial Centers throughout the Island.
In the Hospitality sector, Jose Godinez led the Conceptual Site and Building design efforts of the
now operational Hyatt Place Hotels and supporting facilities in Bayamon and Manati, PR.
Presently, Jose Godinez works as the principal at JLGArchitecs, leading and collaborating with a
group of experience d professionals sha ring resources to easily adapt and work on both small and

large-scale commercial, industrial, and residential projects. The cumulative team experience
joined at JLGArchitects benefits from a variety of work experiences on all sectors of the
construction industry from small to large scale residential projects to complex large commercial
and retail construction programs.
Among the projects that are currently in progress by JLGArchitects, is the initial conceptual design
and program development of a specialized Hospital facility planned for the south of the island
(PR), and the design and production of construction documents for CASAS O L; an Affordable
Housing Prototype mo_d el carefully designed and formulated in close collaboration with a local
contractor and building systems supplier, in respond to the immediate need of well thought-out,
affordable, weather appropriate dwelling units as part of the ongoing recovery efforts after the
impact of Hurricane Maria in 2017 .
Other projects by JLGArchitects include the hurricane damage repairs and improvements to the
Burlington Department Store in Caguas, PR, a collaboration with Aireko Construction, LLC, the
initial project conception and site design phase of the Pep Boys Auto expansion program in the
island (a collaboration with SM+A), specialized Industrial projects at the Pharmaceuticals Lilly def
Caribe and Boehringer lngelheim in Manati, PR, several Residential post hurricane repairs and
improvements projects, and the hurricane damage repairs and improvements to the Plaza
Carolina Mall vertical circulation facilities and Fire Alarm System replacement.
