1. Caso Clínico
EXÁMEN BIOQUÍMICO
• creatinina de 1,89 mg/dl (filtrado glomerular 37 ml/min/1,73m2).

2. DIAGNÓSTICO DIFERENCIAL
Por la presencia de muchas patologías que cursan con disminución del nivel de conciencia, se
realizó un diagnóstico diferencial incluyendo aquellas causas que se acompañen de hidrocefalia
en su evolución.
Por tanto, las entidades que podríamos tener en cuenta en el diagnóstico diferencial serían las
siguientes:
• Sarcoidosis
• Meningoencefalitis bacteriana
• Meningitis fúngica
• Meningitis tuberculosa
3. EVOLUCIÓN
Se hizo un tratamiento de ceftriaxona, vancomicina, ampicilina, aciclovir y dexametasona. Dada
la situación clínica de gravedad del paciente, se le trasladó a UCI donde finalmente precisa
intubación orotraqueal.
RESULTADOS DE EXÁMENES
• Bioquímica del líquido cefalorraquídeo: Muestra un líquido claro, con 21 leucocitos (89% de
mononucleares), consumo de glucosa (20 mg/dl) y proteinorraquia (169 mg/dl).
A pesar de las medidas tomadas, el paciente continúa con deterioro progresivo desde el punto
de vista neurológico y fallece casi dos meses después de haber iniciado el tratamiento
antituberculostático.
4. DIAGNÓSTICO FINAL
Síndrome confusional agudo con disminución del nivel de conciencia secundario a meningitis
tuberculosa en paciente inmunocompetente.

ABSTRACT
A 79-year-old male patient, without drug allergies, with massive pulmonary thromboembolism
and suspected testicular tumor. As usual treatment, the patient took apixaban, amlodipine,
tamsulosin, and omeprazole. He was taken to the emergency room after finding him on the floor
with sphincter incontinence, days before the patient presented continuous nausea, abdominal
pain. Tuberculous meningitis was diagnosed and antituberculostatic treatment was started with
ceftriaxone, vancomycin, ampicillin, acyclovir, and dexamethasone. The clinical case presented
is important since the patients who present this syndrome are mostly older adults or people who
present tuberculosis infection, for which treatment should be administered given the patient's
clinical situation. Acute confunctional syndrome is characterized by a change in mental status,
with inattention and altered levels of consciousness, it is acute and reversible, people who
present this syndrome carry other serious cognitive alterations, mood, sleep and behavior.
Tuberculous meningitis appears in 1% of cases of meningitis and in 5% of extrapulmonary
manifestations in immunocompetent patients. In the patient's case, none of these predisposing
factors could be identified, which leads us to believe that it was tuberculous meningitis in an
immunocompetent patient. Hydrocephalus in patients with tuberculous meningitis is a serious
complication that impoverishes the prognosis.

3. Recomendaciones
El paciente debe seguir el tratamiento dado ,de acuerdo al diagnóstico médico que se le
realizó ,seguir los cuidados recomendados .
