Brief Incident Description

A las 12:40 p.m. se culmina las actividades de limpieza
y desate mecanizado del frente. Los operadores
bloquean la labor y se retiran a tomar su almuerzo. A
las 02:10 p.m. el maestro mina Elmer Meza Chavez
ingresa a supervisar la labor, encontrando
desprendimiento de roca y falla del sostenimiento
pesado (shotcrete – malla – perno – shotcrete) en el
hastial izquierdo de una longitud de 1.5 m
(aproximadamente 15 ton) desde último tramo
sostenido con dirección hacia el ingreso de la labor.
El maestro mina procede a bloquear el ingreso a la
labor e informa al supervisor de turno.

Consequences
 Paralización del subnivel 600 Veta Andalucía Nivel 3900

Immediate Actions
 Se bloquea y paraliza todas las actividades programadas de la labor
 Se realizó la comunicación del evento a la Gerencia General de Operaciones.
 Se conformó un equipo multidisciplinario con los responsables de las Áreas involucradas (Mina, Planeamiento y Geomecanica),
quienes se constituyeron a la labor.
 Se inicia la investigación con la metodología ICAM.
