GENERALIDADES:

Contrato de ejecución de obra : N°005-2017- A / MDT/CONSULTORIA. (28/06/2017)
Consultor de Supervisión : Ing. Edwin G. Porras Vilcapoma
Monto De Contrato De Supervisión : S/. 42,146.35 SOLES
Plazo de Ejecución : 140 Días Calendarios.
Periodo : diciembre-2017
I. ANTECEDENTES:

 Que, mediante CONTRATO N° 005-2017-A-MDT/CONSULTORIA, la Municipalidad Distrital de Ticlacayan suscribe Contrato con el Ing.
Efraín Vidal, RODRIGUEZ FALCON; con el objetivo para la supervisión de la obra “INSTALACION DE AGUA POTABLE Y DISPOSICION
DE EXCRETAS EN EL ANEXO DE NUEVA AURORA, DISTRITO DE TICLACAYAN – PASCO – PASCO”, por un monto que asciende a
S/. 42,146.35 (Cuarenta y dos mil ciento cuarenta y seis 35/100 Soles), por la modalidad de Adjudicación Simplificada N° AS N° 001-2017-
MDT/SUPERV – PRIMERA CONVOCATORIA, por un plazo de 120 días calendarios.
 Que, mediante CARTA N 021-2017/ING.EGPVSUP/CT. recepcionada por mesa de partes con fecha 29 de Diciembre del 2017, el Ing. Edwin
G. Porras Vilcapoma supervisor de la
 obra: “INSTALACION DE AGUA POTABLE Y DISPOSICION DE EXCRETAS EN EL ANEXO DE NUEVA AURORA DISTRITO DE
TICLACAYAN – PASCO – PASCO”. Presenta el Informe Mensual de Supervisión N°05 -diciembre 2017 con un avance al 94.77%.
 Con CARTA N°023-2017/ING.EGPVSUP/CT, presenta solicitud de pago de supervisión del mes de diciembre.

II. CONCLUSION

 Por lo antes indicado se otorga la conformidad de pago, al Ing. Edwin G. Porras Vilcapoma, el pago de Supervisión 05, con el avance
acumulado al 94.77% de la obra “INSTALACION DE AGUA POTABLE Y DISPOSICION DE EXCRETAS EN EL ANEXO DE NUEVA
AURORA DISTRITO DE TICLACAYAN – PASCO – PASCO en la cual por facturar al contratista es:
