PRLOGO
Seis aos han transcurrido desde la pasada edicin. Los continuos avances en la
mejora del conocimiento y tratamiento de la enfermedad imponen renovar y actua-
lizar las pasadas recomendaciones. La presente edicin contina la filosofa de la
anterior: elaborar una gua prctica, independiente y consensuada entre el mayor
nmero posible de expertos provenientes de las diversas sociedades cientficas
espaolas implicadas en la asistencia de la enfermedad, y que cuente con el apoyo
de la industria farmacutica espaola. Esta nueva GEMA, la GEMA 2009, no es una ree-
dicin actualizada de la anterior, sino que se trata de una nueva gua, con diseo, con-
tenidos y metodologa de trabajo diferentes. Entre otras novedades se ha contado
con la ayuda tcnica del Centro Cochrane Iberoamericano y en representacin de las
personas afectadas por la enfermedad, con el apoyo del Foro Espaol de Pacientes.
Esta nueva GEMA 2009 no petende aglutinar todo el conocimiento disponible sobre
la enfermedad. No se trata de una monografa sobre asma. Ha sido diseada para

Por su envergadura, participantes, diseo y contenido, esta GEMA 2009 nace con
las mejores perspectivas de xito. Estamos convencidos de que va a suponer un
referente a seguir en cuanto a consensos multidisciplinares espaoles realizados, y
sobre todo confiamos en que todo este esfuerzo se derive en una mejora en la for-
macin y actuacin de nuestros profesionales, y en consecuencia se traduzca en
un mejor control de la enfermedad y de la calidad de vida de las personas que la
padecen, razn de ser y principal objetivo de esta GEMA 2009.

No obstante, los redactores de la presente gua entendemos que si bien dicha clasi-
ficacin es muy til para categorizar la evidencia dirigida a evaluar la eficacia terapu-
tica de frmacos u otras acciones, minusvalora otros estudios tan importantes como
los que versan sobre la eficacia diagnstica de algunas exploraciones o los de datos
epidemiolgicos. De ah que en la presente gua muchas de las evidencias disponibles
para evaluar los importantes estudios que determinaron la idoneidad de algunas
exploraciones diagnsticas hayan sido calificados con un nivel de evidencia C.

Una vez clasificada la calidad (confianza en los resultados) de los estudios disponibles
se debe graduar la fuerza de las recomendaciones, entendiendo por este concepto
la confianza en que el cumplimiento de determinada recomendacin se traducir en
ms beneficios que en riesgos. Para establecer la fuerza de las recomendaciones se
ponderaron, por tanto, la calidad de la informacin (a partir de la clasificacin men-
cionada), el balance entre los riesgos y los beneficios de las intervenciones, los cos-
tes as como los valores y preferencias de los pacientes. Posteriormente se clasifica-
ron las recomendaciones en dos tipos, fuertes y dbiles (a favor o en contra). Las
recomendaciones fuertes (recomendaciones R1) representan aqullas en las que
el grupo elaborador confa en que conlleven ms beneficios que riesgos. Para este
tipo de recomendaciones se ha redactado el texto utilizando expresiones tales
como se recomienda o se debera. Las recomendaciones dbiles (recomenda-
ciones R2) representan aqullas en las que existe incertidumbre sobre si su aplica-
cin conllevar ms beneficios que riesgo, y su redaccin incluye expresiones tales
como se podra considerar o se podra valorar.

Epitelio bronquial: est daado, con prdida de las clulas ciliadas y de clulas secreto-
ras. El epitelio libera mediadores que fomentan la inflamacin. Agentes contaminantes e in-
feccin por virus respiratorios pueden estimular su produccin y daar el epitelio. El proce-
so de reparacin que sigue al dao epitelial suele ser anormal, aumentando las lesiones
obstructivas que ocurren en asma21.
Musculatura lisa bronquial: contribuye a la obstruccin por su hipertrofia, contraccin y
produccin de mediadores proinflamatorios similares a los de las clulas epiteliales.
Clulas endoteliales: en la circulacin bronquial participan en el reclutamiento de clulas
inflamatorias desde los vasos a la va area mediante la expresin de molculas de adhesin.
Fibroblastos y miofibroblastos: estimulados por mediadores inflamatorios y factores de
crecimiento, estn implicados en la remodelacin de la va area.
Nervios colinrgicos de la va area: se pueden activar, causar broncoconstriccin y se-
crecin de moco. Los nervios sensoriales pueden causar sntomas como la tos y la opre-
sin torcica y pueden liberar neuropptidos inflamatorios22.

Contraccin excesiva del msculo liso de la va area. Puede resultar del aumento del
volumen y/o de la contractilidad de las clulas del msculo liso bronquial.
Desacoplamiento de la contraccin de la va area como resultado de la inflamacin bron-
quial. Puede conducir a un excesivo estrechamiento y a una prdida del umbral mximo de
la contraccin cuando se inhalan sustancias broncoconstrictoras.
Engrosamiento de la pared de la va area. Amplifica el estrechamiento debido a la con-
traccin del msculo liso bronquial por razones geomtricas.
Nervios sensoriales sensibilizados. Por la inflamacin pueden llevar a una broncoconstric-
cin exagerada en respuesta a los estmulos sensoriales.

Diagnstico mdico de asma en alguno de los padres.

El diagnstico de asma debe basarse en medidas objetivas de afectacin

funcional. La espirometra es la prueba de eleccin.

Otras pruebas que pueden ser tiles en el manejo del nio preescolar con
asma son la oscilometra forzada de impulsos (IOS), la medida de las resis- D
tencias por oclusin (Rint), el anlisis de la curva flujo volumen a volumen
corriente o la medida de resistencias por pletismografa, aunque suelen
utilizarse en laboratorios especializados. Recientemente se ha publicado
la normativa ATS/ERS sobre funcin pulmonar en el nio preescolar58. En
el lactante la tcnica ms utilizada es la compresin rpida toracoabdomi-
nal. Las pruebas basales de funcin pulmonar son poco tiles para el
diagnstico de asma en preescolares, siendo ms til demostrar una res-
puesta broncodilatadora o una prueba de provocacin bronquial median-
te alguna de las tcnicas mencionadas.

Se recomienda realizar una espirometra con prueba broncodilatadora
para confirmar el diagnstico de asma y valorar objetivamente su grave- R2

En el asma persistente se aconseja evaluar el potencial papel de los aero-
alrgenos mediante valoracin clnica y pruebas de prick o IgE. Es impor- R2

En los casos de asma alrgica mal controlada con dosis altas de glucocorti-
coide y agonista 2 adrenrgico de accin larga se puede aadir el anticuer- A
po monoclonal anti-IgE (omalizumab) por va subcutnea, que mejora los sn-
tomas diarios130 y disminuye las exacerbaciones131-133, incrementando el
control global de la enfermedad.

Los frmacos para el asma se pueden administrar por distintas vas (oral,
inhalatoria y endovenosa), pero las ventajas de la va inhalatoria hacen que C
sta sea la de eleccin136,137. Los dispositivos de inhalacin ms habituales
son el inhalador presurizado, que se puede utilizar con o sin cmara espa-
ciadora (Volumatic, Aerochamber, Aeroscopic, Babyhaler, Nebucham-
ber, Inhalventus, Prochamber, Optichamber e Ildor), el inhalador pre-
surizado con solucin de partculas extrafinas (Modulite), los dispositivos
de polvo (Turbuhaler, Accuhaler, Aerolizer, Novolizer, Handihaler,
Easyhaler y Twisthaler) y los nebulizadores (jet o ultrasnicos), cada uno
con caractersticas diferenciales que deben considerarse al prescribir-
los138,139. La edad y la destreza del paciente son los factores ms importan-
tes para elegir el dispositivo ms adecuado en cada caso. La utilizacin de
hidrofluoroalcanos (HFA) como propelente en los inhaladores presurizados
aumenta el depsito pulmonar140, por lo que su utilizacin puede afectar a
las dosis, que debern ser ajustadas segn las indicaciones del fabricante.

RECOMENDACIONES
En pacientes con sntomas de asma, y en cualquiera de los escalones tera-
puticos, se recomienda utilizar un agonista 2 adrenrgico de accin corta R1
a demanda para el alivio rpido de los mismos.

asma por epitelios (retirar los animales del domicilio)149-152 y cuando stas
se instauran de forma precoz en la evolucin de la enfermedad (ver asma
ocupacional en el captulo 7.3).
Algunos pacientes asmticos, en especial los que cursan con poliposis naso-
sinusal, pueden presentar broncoespasmo cuando se les administra aspirina
C
u otros antiinflamatorios no esteroideos (AINE). Muchas de estas reacciones
son graves o incluso mortales160, por lo cual es necesario que los pacientes
estn correctamente diagnosticados, bien por una historia clnica evidente
(varias reacciones a distintos AINE) o mediante provocacin oral, que puede
ser sustituida en los casos graves por la provocacin inhalatoria bronquial o
nasal161,162. La mejor alternativa como analgsico para estos pacientes (tabla
3.4) es el paracetamol administrado en dosis inferiores a 650 mg por toma,
puesto que algunos pacientes pueden presentar crisis de broncoespasmo,
con mayor frecuencia si se utilizan dosis elevadas. Los opiceos, como el tra-
madol o la codena, tambin se consideran analgsicos alternativos seguros.
Como frmacos antiinflamatorios se pueden emplear los glucocorticoides.
Los inhibidores selectivos (meloxicam) o especficos (celecoxib, etoricoxib)
de la COX-2 pueden ser otra alternativa, si bien antes de recomendarlos es

Los pacientes con planes de accin por escrito para tratar crisis leves, con
registros domiciliarios de PEF, utilizan la mejor estrategia en el manejo de C
exacerbaciones leves, ya que la pueden aplicar precozmente204. Para ello
tienen que estar entrenados para reconocer los indicadores precoces de
una exacerbacin y actuar de forma inmediata segn el plan de accin que
tengan sealado, que tiene que incluir las medidas que se van a tomar,
dependiendo de la respuesta al tratamiento.

Diversos estudios han constatado que, una vez alcanzada la meseta de res-
puesta mxima broncodilatadora, la administracin de ms medicacin no C
se traduce en una mayor mejora clnica213,225. Por dicho motivo, habitual-
mente se recomienda decidir la necesidad de ingreso hospitalario del
paciente a las tres horas de haberse iniciado el tratamiento. Aquellos pacien-
tes que han recibido un tratamiento adecuado durante dicho plazo y perma-

necen sintomticos, que requieren oxigenoterapia para mantener una SaO2
superior al 90% y que muestran una reduccin persistente de la funcin pul-
monar (FEV1 o PEF inferior al 40%), deben ser hospitalizados226.
