Tests for glandular fever

This infection is caused by Epstein-Barr virus (EBV). Infected cells produce so-called
heterophile antibodies (these are IgM molecules that agglutinate horse and sheep RBCs but do
not agglutinate ox RBCs and do not react at all with guinea-pig RBCs).
There are various kits available that can detect the presence of heterophile antibodies
and in the right clinical context will confirm a diagnosis of EBV infection. The Monospot test is
probably the commonest in current use. The Paul-Bunnell test was the first to demonstrate the
presence of heterophile antibodies in patients with EBV infection.
Clinical features
Glandular fever often affects young adults (1225 years) and results in malaise, fever,
tonsillitis, petechial haemorrhages on palate and lymphadenopathy. Splenomegaly is fairly
common. A similar clinical picture is seen in CMV, Toxoplasma and early HIV infections.

Pruebas para Fiebre Glandular

Esta infeccin es causada por el virus de Epstein-Barr (VEB). Las clulas infectadas
producen los llamados anticuerpos heterfilos (estos son molculas IgM que aglutinan los
glbulos rojos de caballos y ovejas pero no aglutinan glbulos rojos de buey y no reaccionan en
absoluto con los glbulos rojos de conejillos de india).
Hay varios kits disponibles que pueden detectar la presencia de anticuerpos heterfilos y
en el correcto contexto clnico, confirmar un diagnstico de la infeccin por VEB. La prueba de
mononucleosis es probablemente la prueba ms popular usada actualmente. La prueba de
Paul-Bunnell fue la primera en demostrar la presencia de anticuerpos heterfilos en pacientes
con infeccin por VEB.
Caractersticas Clnicas
La Fiebre glandular con frecuencia afecta a los adultos jvenes (12 a 25 aos) y se
traduce en malestar general, fiebre, amigdalitis, hemorragias locales en paladar y
linfadenopata. La Esplenomegalia es bastante comn. El mismo cuadro clnico se observa en
infecciones por CMV, Toxoplasma y recientemente en infecciones por VIH.
Muestra: Con anticoagulante EDTA
