INCIDENTALOMA ADRENAL
ES MUY COMUN 3 7 % DE LA POBLACION ADULTA FRECUENTEMENTE SON NO HIPERFUNCIONANTES 0,5 AL 5% DE LOS ESTUDIOS TOMOGRAFICOS. ES IMPORTANTE SABER SI NUESTRO PACIENTE EN ESTUDIO ES PORTADOR DE UN TUMOR PRIMARIO PORQUE LA GLANDULAR ADRENAL ES ASIENTO COMUN DE METASTASIS (CARCINOMAS DE PULMON, MAMA Y COLON; TAMBIEN LINFOMAS Y MELANOMAS)

UNA LESION BENIGNA ES HOMOGENEA CON FORMA Y BORDES REGULARES Y ENCAPSULADOS UNA LESION MALIGNA ES TIPICAMENTE INHOMOGENEA CON BORDES IRREGULARES, EVIDENCIA DE INVASION LOCAL O LINFOADENOPATIA ASOCIADA.

METASTASIS Mujer de 51 aos con cncer de pulmn. TC contrastada. (Incidental adrenal lesions: Principles,

LAS LESIONES MENORES A 4 CM SON PROBABLEMENTE BENIGNAS, ASI COMO LAS MAYORES A 4 CM SON PROBABLEMENTE MALIGNA
GENERALMENTE LOS ADENOMAS TIENEN UNA DENSIDAD < 10 UH. LOS CARCINOMAS TIPICAMENTE TIENEN UNA DENSIDAD > 18 UH. TRAS LA INYECCION DE CONTRASTE LOS ADENOMAS PRESENTAN UN REALCE IMPORTANTE (< 100 UH), CONJUNTAMENTE CON OTRAS LESIONES ADRENALES. LA DIFERENCIA RADICA EN EL WASH-OUT.
FRMULA PARA PORCENTAJE ABSOLUTO DE WASHOUT (UH contrastadas UH a los 15 min)/(UH contrastadas UH no contrastadas) Un valor > 60 % es diagnstico de adenomas.
(UH contrastadas UH a los 15 min)/UH contrastadas X 100 Un valor > 40 % es diagnstico de adenomas.

ACORDARSE, DE NUESTRO INFORME DEPENDERAN LOS SIGUIENTES PASOS EN RELACION AL DIAGNOSTICO Y TRATAMIENTO DEL PACIENTE. DEBEMOS RECOMENDAR CMO DEBERA SER MANEJADO EN SU SEGUIMIENTO, DIAGNOSTICO Y TRATAMIENTO.
