We usually say (Generalmente decimos): Where are you from? OR Where do you come from?
Answers (Respuestas):

Ciudad: TO BE + FROM + CITY Example: I am from Hohenau/ I´m from Hohenau.


País: TO BE + FROM + Example: I am from Paraguay/I´m from Paraguay.
COUNTRY
Nacionalidad: TO BE + NATIONALITY Example: I am Paraguayan/I´m Paraguayan.
