1. I hereby certify that within the last ten years:

a. I have not been convicted of a violation of, or a conspiracy to violate, any law
or regulation of the United States, or of any other country, concerning
narcotics, psychotropic drugs, or other controlled substances.

b. I am not and have not been an illicit trafficker in any such drug or controlled
substance.

c. I am not and have not been a knowing assistor, abettor, conspirator, or

colluder with others in the illicit trafficking in any such drug or substance.

2. I understand that the United States Department of State (DOS) may terminate
my training if it is determined that I engaged in the above conduct during the last
ten years or during my DOS-funded training.

OFENSAS CON ESTUPEFACIENTES Y TRÁFICO DE DROGAS

1. Por medio de la presente certifico que en los últimos 10 años:

a. No he sido condenado por una violación de, o una conspiración para violar
cualquier ley o regulación de los Estados Unidos, o de cualquier otro país, en
relación con estupefacientes, drogas psicotrópicas u otras sustancias
controladas.

b. No soy y no he sido un traficante ilícito de ninguna droga o sustancia

controlada.

c. No soy y no he sido un encubridor, ayudante, cómplice, instigador o coludido

con otros en el tráfico ilícito de ninguna droga o sustancia controlada.

2. Entiendo que el Departamento de Estado de los Estados Unidos (DoS) puede dar
por terminado mi entrenamiento, si se determina que he participado en alguna
de las conductas anteriormente mencionadas durante los últimos diez años o
durante mi entrenamiento financiado y apoyado por el DoS.
