La Sra. Rebeca Elizabeth Ojeda Celis C.C. 1.043.612.609 es mi paciente y está
bajo mi cuidado profesional. Estoy íntimamente familiarizada con su historia y
con las limitaciones funcionales impuestas por su trastorno depresivo mayor,
tal como se define The Diagnostic and Statistical Manual of Mental Disorders,
Fifth Edition (DSM-5). Ella cumple con la definición de discapacidad, y está
protegida bajo la Ley de Protección de la Salud (The Fair Housing Act) y la Ley
de Acceso al Transporte Aéreo (The Air Carrier Access Act).
Debido a su enfermedad mental, la Sra. Rebeca Elizabeth Ojeda Celis tiene
ciertas limitaciones con respecto a la depresión y la ansiedad que limitan
sustancialmente una o más de sus tareas más importantes en la vida. Para
ayudar a aliviar estas dificultades, le estoy recetando a la Sra. Rebeca Elizabeth
Ojeda Celis, un animal de apoyo afectivo (un perro). Este animal de apoyo
afectivo es necesario para que la salud de la Sra. Rebeca Ojeda pueda
permanecer de forma independiente en su hogar, y/o para la compañía
terapéutica durante los vuelos en avión.

Mr. Jose Cuellar Anastasiu C.C 79.488.569 is my patient and under my professional
care. I am intimately familiar with his history and with the functional limitations
imposed by his Major Depressive Disorder as defined in The Diagnostic and
Statistical Manual of Mental Disorders, Fifth Edition (DSM-5). He meets the definition
of disability, and is protected under the The Fair Housing Act, and The Air Carrier
Access Act.

Due to mental illness, Mr. Jose Cuellar Anastasiu has certain limitations regarding
coping with depression and anxiety that substantially limits one or more of his major
life activities. In order to help alleviate these difficulties, I am prescribing Mr. Jose
Cuellar Anastasiu an emotional support animal (One dog). This emotional support
animal is necessary for to Mr. Jose Cuellar Anastasiu health to live independently in
hir home, and/or for therapeutic companionship during airplane flights.
