Antecedentes y propsito las caracteristicas de los infartos restringidos a la mdula dorsal han recibido poca atencin.
Este estudio tuvo como objetivo definir las distintas caractersticas clnicas del infarto medular dorsal.
Mtodo De los 172 pacientes con diagnstico de infarto medular en el National University Bundang Hospital de 2003 a
2014, 18 pacientes con infarto medular dorsal aislado fueron sometidos a anlisis de hallazgos clnicos y de laboratorio.
ResultadosTodos los pacientes presentaron un sndrome vestibular agudo aislado con mareos/vrtigo y desequilibrio
Casi todos los pacientes (17/18, 94%) mostraron signos de las implicaciones de ncleo vestibular, ncleo prepositus
hypoglossi, o inferior cerebellar peduncle, que inclua nistagmo evocado por la direccin (n=12), pruebas negativas de
impulso de cabeza (n=8), skew deviation (n=7), nistagmo central al agitar la cebza (n=6), y nistagmo espontaneo (n=2).
Las imgenes de resonancia magntica inicial incluian las imgenes ponderadas por la difusin fueron negativas en 7

pacientes (39%). Doce pacientes (67%) mostraron una progresin y desarrollaron anomalas neurolgicas adicionales ,
pero los resultados neurolgicos fueron favorables.
ConclusionesLa presencia de signos vestibulares centrales permite la diferenciacin en el lecho de la cama del
sndrome vestibular aislado debido a un infarto medular dorsal de los trastornos vestibulares perifricos agudos. Debido
a que las imgenes de resonancia magntica falsas negativas y la progresin subsiguiente son frecuentes en el infarto
medular dorsal, el reconocimiento temprano a travs de la evaluacin es importante para las gestiones adecuadas.

Sujeto
De los 172 pacientes con diagnstico de infarto medular en el Hospital
from 2003 to 2014, 18 pacientes (8%) tenan un infarto restringido a la
medula dorsal (14 men: age range, 3373 years; mean ageSD, 6112)
basados en las caracteristicas clnicas y los hallazgos de la resonancia
magnetica (MRI). El intervalo desde el inicio de los sntomas hasta la
evaluacin oscil entre 0 y 10 das (median, 1 day). Todos los pacientes
tuvieron una evaluacin neuro-otolgica (J.-S.K). Sobre la base de los
hallazgos de estudios previos, hemos sido capaces de clasificar las
caracteristicas clnicas observadas en nuestros pacientes en 4 tipos de
acuerdo a la estructura o estructuras afectadas VN, NPH, ICP, and
mixed.9,12,13
Este estudio sigui los principios de la declaracin de Helsinki y se

Neurotologic Evaluation

Results
Clinical Characteristics
Los 18 pacientes presentaron sndrome vestibular agudo
aislado (AVS) caracterizado por vrtigo o inestabilidad que
fue el nico hallazgo en 15, y se asoci con tinnitus en el
odo ipsilateral (n=2, patients 7 and 11) o contralesional
(n=1, patient 12) en los 3 pacientes restantes.
Cuatro pacientes (patients 6, 7, 9, and 12) reportaron un
vrtigo espontneo predente de 1 minuto a 6 horas con (n=2)
sin (n=1) tinnitus de 2 semanas a 4 meses antes del infarto.
Dos de ellos tenian resonancia magnetica cerebral dentro de
las 6 horas de episodio anterior, que eran normales

of print on October 13, 2015, and appeared in the November 2015 issue of the journal, a
correction was needed.
The term head impulse, nystagmus, test-of-skew has been changed to read, Head-Impulse
NystagmusTest-of-Skew throughout the article.
On page 3082, in the last paragraph before the Clinico-Anatomic Correlation section and on
page 3085, in the last paragraph before the Lesion Location and Involved Structure(s) section
there were minor errors in the numbers. As indicated in the table, the number of patients with
positive Head-ImpulseNystagmusTest-of-Skew is 16/1, 89%.
