contraccin muscular, cefalea por tensin, cefalea tensional, cefalea de tensin, cefalea por estrs, cefalea
ordinaria, cefalea por tensin psquica, cefalea esencial,
cefalea idioptica, cefalea psicomiogentica, cefalea
psicgena y cefalea psicogentica.
Observacin: esta forma de cefalea, extraordinariamente frecuente y muy difcil de diferenciar de la jaqueca, aparece sin prdromos; habitualmente es bilateral,
occipital o frontal, y puede tener carcter pulstil, de
presin o sordo. Se acompaa de ansiedad e hiperexcitabilidad, fotofobia, raras veces de nuseas y excepcionalmente de vmitos. Se asocia a la fatiga, a excesos de
comida o de bebida y a situaciones pasajeras de estrs
laboral o psicosocial. La IHS, en su Clasificacin de
1988, adopt los criterios consensuados de los expertos
en cefalea de un comit internacional, dio por tierra con
todas las antiguas denominaciones y las sustituy por
tension headache, puesto que algunas (psicogentica,
por tensin psquica, por contraccin muscular)
sugieren un mecanismo fisiopatolgico que an no se
ha demostrado. En la 2. edicin de la Clasificacin
figura como tension-type headache y se divide en cuatro
categoras, a saber: cefalea episdica infrecuente de tipo
tensional (infrequent episodic tension-type headache),
cefalea episdica frecuente de tipo tensional (frequent
episodic tension-type headache), cefalea crnica de tipo
tensional (chronic tension-type headache) y probable
cefalea de tipo tensional (probable tension-type headache).
