RESUMEN
En Inmunohematologa se ha desarrollado una amplia gama de procederes de
deteccin e identificacin de anticuerpos eritrocitarios in vitro, por lo cual se
realiza una revisin de tcnicas y mtodos empleados con este objetivo, como
son el mtodo que utiliza eritrocitos pretratados con enzimas proteolticas y las
tcnicas de Polibreno, que utiliza solucin de baja fuerza inica (LISS), la de
antiglobulina indirecta, la de aglutinacin en gel, la inhibicin de la aglutinacin,
la hemlisis y la adherencia de eritrocitos en fase slida. Se abordan los problemas
que afectan a la reaccin de aglutinacin entre el antgeno y el anticuerpo; para
una mejor comprensin la reaccin de aglutinacin se subdivide en su primera y
segunda etapa. En la primera etapa los factores que se analizan son concentracin
de antgeno y anticuerpo, pH, temperatura, fuerza inica y tiempo de incubacin;
en la segunda etapa la caracterstica del anticuerpo, localizacin y nmero de
sitios antignicos, fuerzas que mantienen la distancia entre los eritrocitos, uso de
la albmina bovina, uso de enzimas, efecto de dosis y efecto de molculas con
carga positiva.

La alteracin antgeno-anticuerpo
puede verse en diferentes contextos; uno
de estos es la reaccin de aglutinacin de
los eritrocitos, que se discutir a continuacin, pues es el fenmeno que por lo
general ocurre en la mayora de las tcnicas
que se realizan en Inmunohematologa.
Existen 2 requisitos para que la
reaccin antgeno-anticuerpo se produzca:
uno es la adecuada complementariedad de
encaje, podrn unirse a los anticuerpos solo
aquellos antgenos con determinantes
antignicos que se ajusten al sitio de

El pesquisaje de anticuerpos
eritrocitarios tiene como objetivo detectar
y luego identificar anticuerpos clnicamente
significativos, que puedan causar reaccin
transfusional y acortamiento de la sobrevida
normal de los eritrocitos, de modo que
deben emplearse mtodos in vitro
apropiados.
Usualmente se emplean mltiples
tcnicas para la deteccin de anticuerpos
eritrocitarios, una de ellas es el tratamiento
enzimtico de los eritrocitos con proteasas.
Esta tcnica es mucho ms sensible que
otras en las que no se utilizan eritrocitos
pretratados enzimticamente, especialmente
en la deteccin de anticuerpos Rh y otros
anticuerpos que slo reaccionan ante
eritrocitos pretratados.18
A pesar de las ventajas que brinda el
uso de enzimas proteolticas, es importante
alertar en relacin con su uso como rutina
en la deteccin de anticuerpos eritrocitarios.
Los eritrocitos pretratados con enzimas
pueden detectar anticuerpos fros y otros
anticuerpos que no son clnicamente
significativos, adems existe muy poca
correlacin entre la presencia pretransfusional de anticuerpos detectables
nicamente por mtodos enzimticos, y el
desarrollo de anticuerpos demostrables por
otros mtodos, das o semanas despus de
la transfusin.1,19
Dos de las tcnicas que deben
emplearse como rutina en la evaluacin de
anticuerpos eritrocitarios son: el mtodo del
polibreno y el de LISS (del ingls low ionic

SUMMARY
Awide range of procedures for the detection and identification of red cell antibodies
in vitro has been developed in Immunohematology. Therefore, it is made a
review of the techniques and methods used with this aim, such as the method
using erythrocytes pretreated with proteolytic enzimes and the techniques of
Polibreno that utilize low ionic force solution (LIFS), the indirect antiglobulin
test, the gel agglutination test, the agglutination inhibition test, hemolysis and
the solid phase erythrocyte adherence test. The problems affecting the
agglutination reaction between the antigen and the antibody are also dealt with.
The agglutination reaction is subdivided into its first and second phase for a
better understanding. In the first phase, the antigen and antibody concentration,
pH, temperature, ionic force and incubation time are analyzed. The characteristic
of the antibody, localization and number of antigenic sites, forces maintaining
the distance among the erythrocytes, use of bovine albumin, use of enzimes,
dose effect and effect of molecules with positive charge are studied in the second
phase.
