Medical Scenario #4

 P: Good Evening my name is Daniel I am the resident on call tonight. What brings you to the
ER?

 L: Hola doctor mi nombre es Mercedes Isabel Ordoñez y soy la hija de Don Rufino lo traigo
porque estoy bien preocupada doctor hace varios meses que yo no lo venía a visitar porque
trabajo en otro estado. Hoy que vine a visitarlo lo veo amarillo, dice que le duele mucho el
estómago al ratito vomito y no quiere comer nada cuando él siempre ha sido bien comelon.

 P: I see what other symptoms do you have Mr. Ruffino? Nausea, Vomit?

 L: Si siento muchas nauseas después de comer por eso es que ya días no estoy comiendo mucho
porque no me siento nada bien.

 P: I understand Mr. Ruffino, and have you noticed any unintentional weight loss in the last
months?

 L: Si doctor bastante he bajado como 3 tallas de pantalón, también he notado que la parte blanca
de los ojos se me está poniendo amarilla.

 P: Ok, do you have any itching in your skin?

 L: Si doctor tengo bastante picazón también no se a que se deberá eso.

 P: Well, Mr. Ruffino usually when you notice a yellowish color in the sclera -that is the white
part of the eye- it is usually a sign of jaundice that is caused by an increase of the bilirubin
levels.

 L: ¡OH! No sabía eso doctor.

 P: Yes, Mr. Ruffino to confirm that we just need to do blood work in order to check you
bilirubin levels. Also tell me have you had any abdominal pain?

 L: Si doctor ya van varios meses que siento este dolor en la panza.

 P: And where is this pain located? In a scale of 0 to 10, 0 being no pain at all and 10 being the
worst pain ever how would you describe this pain?

 L: Pues es por todo esta parte mire doctor y pues yo le daria un 8 de 10 al dolor doctor porque si
es bien fuerte y no se me quiere ir. Tambien he notado que por ratos este dolor se me pasa a la
espalda tambien.

 P: For what you are telling me Mr. Ruffino this belt like pain that irradiates to your back sound
like the pancreatic pain which could also explain the jaundice. Hmmmm, let me feel your
stomach.
 L: Si doctor haga lo que tenga que hacer. ¿Me destapo la panza?

 P: Yes, please uncover your belly. Tell me if you feel any pain.

 L: Ok doctor.
 P: Do you feel pain in here?

 L: ¡Ay! ¡Ay! ¡Ay! ahí si doctor.

 P: And here?

 L: ¡Ay! ¡Ay! ¡Ay! Si también doctor duele.

 P: And here?

 L: ¡Ay! ¡Ay! ¡Ay! también doctor mucho duele.

 P: Hmmm, let me try to feel your gall bladder and tell me if it hurts.

 L: Esta bien doctor yo le digo.

 P: Do you feel any pain?

 L: No doctor no siento ahí fíjese.

 P: Hmmm, have you noticed any dark color urine and white color feces?

 L: Fíjese que si doctor la semana pasada hasta me asuste porque el pupu era blanquito y la orina
me salió bien oscura como Coca-Cola doctor.

 P: I see Mr. Ruffino I was able to feel your gall bladder and you told me that it didn't hurt when
usually when it becomes enlarged it should be painful. This sign is called the Courvoisier
Terrier's Law which consists of a palpable enlarged gallbladder that is nontender and mild
jaundice. Not to scare you Mr. Ruffino but we might be facing a cancer in the head of the
pancreas.

 L: ¡Dios mío! ¿Me voy a morir doctor?

 P: Don't panic Mr. Ruffino we still need further testing to have a certain diagnosis. I will order a
gall bladder biopsy, CT scan of the pancreas, some blood work and tumoral markers test to
confirm. So, for now we will admit you to the hospital and give you some pain medication and
as soon as we have any results back, I will be giving you updates of what we've found.

 L: Esta bien doctor. Primero Dios no es eso.

 P: Yes, Mr. Ruffino we will take very good care of you. For now, I would like you to rest and
tell me if you notice any other symptom. Ok?

 L: Esta bien doctor.
