a FLAMBOYAN MEMORANDUM OF UNDERSTANDING BETWEEN FLAMBOYAN FOUNDATION PR AND THE UNIVERSITY OF PUERTO RICO AT CAROLINA, Flamboyan Foundation PR (Flamboyan), serving as fiscal agent for the Puerto Rico Donors’ Education Collaborative (PRDEC), and the University of Puerto Rieo Carolina ‘campus (UPR Carolina) agree toenter into partnership from September 2035-Jume 2016 with the purpose of successfully retaining incoming first-year students from Lotza’s Escuela Superior Carlos Escobar Lopex and Nueva Superior Voeacional high schools. UPR Carolina's Centro Universitario para el Acceso (CUA) will establish a college mentoring program to provide support and guidanee for the thirty-five (35) incoming students that will form the Lotza cohort, enabling and supporting their continued eollege 1, Objectives | Support incoming first-year students from Lofea high echools in the process of ‘adapting to college life from the beginning. Collaborate with the students in the development oftheir success plan, Offer workshops aimed at strengthening and developing skills needed for college 4. Monitor participating students’ progress and make referrals to acades servioes as needed. 5. Count with a scholarship fund to cover miscellaneous student needs such as ‘transportation, books, and uniforms. 66. Establish recurring and consistent communications between CUA and PRDEC to monitor the cohort’s progress. support M11, Outcomes ‘The major outeome for this partnership is to ensure the successful retention ofthe thisty- five (35) incoming first-year students from Lotza high schools, Desired outeomes: 4, 200% of incoming Lofea students admitted to UPR Carolina participate in CUA's college mentoring program. 2 3 + 100% of the Loiza cohort has 2 mentor-student relationship where they receive support and consistent follow-up, and benefit from the other program components. Over 80% ofthe Lofea cohort continues in college after the first year (2015-2036). Over 70% of the Lofza cohort has aerdemic progress after the first year (2015- 2016), IV. Roles and Responsibilities — UPR Carolina Toachieve these outcomes, UPR Carolina agrees to deliver the following outputs: 10. Identify and retain a Program Coordinator for the eollege mentoring program. Reach out to incoming Lofza students and convene an initial meeting with all students and PREC steff, Identify, reruit, and train five college mentors (maximum of seven students per mentor for a total of 35 students). Create a detailed action plan with a schedule of meetings, tasks and timeline for ‘the academie year Coordinate and facilitate interactive sessions between students and mentors. - Hold weekly check-in meetings between Program Coordinator and mentors to follow-up on student progress. Collaborate with students towards the development oftheir Success Plan. Offer bimonthly workshops to students aimed at strengthening and developing skills needed for college success, a, ‘Time management . Study habits ©. Research skills 4. Spanish and English writing salle . Oral and written presentation sails ‘Monitor individual and eohort progress and make aeademie support referrals as needed. 4 Develop a tracking and referral database with individual and cohort information . Monitor academfe progress for each term ©. Academic support referral to existing UPR services >. Perform preliminary program evaluations at the end of each 10-week term, ‘Establish reeurring and consistent eommuniations between UPR Carolina's CUA. and PRDEC to monitor Lofea cohort's progress. Provide Flamboyan with retroactive data (2009-2015) for students from Lofza’s high schools. V. Roles and Responsibilities ~ Flamboyan (PRDEC) ‘To achieve these outcomes, Flainboyan agrees to do the| lowing: 4. Establish a scholarship fund in the amount of $7,000.00 (#200 per student per academic year) to be used to cover miscellaneous student needs such as books, ‘uniforms, meals, and transportation. Specific parameters will beset up forthe use of these funds and UPR Carolina's CUA will submita monthly report detailing the use of funds. 2, Maintain recurting and consistent communications between UPR Carolina's CUA ‘and PRDEC to monitor Loiza cohot’s progress. V1. Disbursement of Funds Flamboyan will provide UPR Carolina $44,900.00 to fulfill its lead role in the partnership. Amount will be disbursed in two payments across the one-year grant term: ‘$22,450.00 once this MOU is signed and the remaining balanee of $22,450.00 in January 2016 after reviewing the progress ofthe outputs set forth in Section IV ofthis agreement. ‘VIL, Ownership
