To obtain the total score, just count all the checked items under the Visit 2 column in the form. Fill up Must = by counting how many items are present or checked. Tally how many desired or exemplary items are present and fill in the number in their appropriate cells.

Transfer the scores from the Accreditation Assessment Report (Appendix 7). Take note that the scores are entered in the correct cells for the appropriate columns. Fill up the Accreditation Assessment Report in triplicate.

Level 2: Very Satisfactory - Approximately 50% to 75% of the desired items per focus area for which the service provider is accountable are present in addition to the 179 MUST points for 0-3 year old children and 141 points for 3.1-5.11 year old children. Level 3: Highly Satisfactory-Approximately 80% or more of the desired items per focus area are present in addition to the 179 MUST points for 0-3 year old or 141 points for 3.1-5.11 year old children.
Level 4: Outstanding - Approximately 50% to 75% of the exemplary items per focus area in addition to the 191-192 MUST and DESIRED points for 0-3 year old or 153-154 points for 3.1-5.11 year old children. Level 5: Exemplary - Approximately 80% or more of the exemplary items per focus area in addition to the 191-192 MUST and DESIRED points for 0-3 year old or 153-154 points for 3.1-5.11year old children.
