En 2017 el AO fue incluido Enfermedades Tropicales Desatendidas de la
Organización Mundial de la Salud.
Mundo 1,2 a 5,5 millones de mordeduras/año con 30% de OT (0,4 a 1,8
millon) y 5% de muertes (20 mil a 94 mil muertes)
América 60,000 mordeduras/año con 0,6% de muertes
Colombia Sivigila 2018 registra 4500 mordeduras/año con 0,7% de
muertes
En Colombia hay aprox 300 especies serpientes y solo 51 son venenosas
(17%)

borrosa, diplopia, oftalmoplegia y anisocoria, disfagia
para deglutir y/o masticar, sialorrea,
disminucion del reflejo del vomito, disnea
- Fascies miastenica o inespresiva o neurotóxica de
Rosenfield

ELAPIDICO - CROTALICO
 LABORATORIO

- TP, TPT, INR, CK total, Transaminasas, LDH, Cuadro Hemático, fibrinógeno, DD,
Electrolitos, BUN, Creatinina, Parcial de Orina, Rx Torax y TAC. En Elapidico-
Crotalico GA. Se repiten a criterio clinico

- La prueba del todo o nada: 5 cc sangre venosa fresca en tubo de ensayo de

vidrio, limpio y seco. Homogenizar - reposo gradilla al ambiente x 20 min.
* Si hay Coagulación (es negativa) ~ no hemotoxicidad y no antíveneno

* Remitirse al inserto en cada caso

* Reconstituir suavemente sin sacudir
* Premedicar con corticoide es opcional
* Diluir en 250/100cc SSN adulto/niño
* Iniciar lento ~ 10 a 50 cc/h x 5 a 10 min
* Si no hay anafilaxia pasar en 1 hora
* En anafilaxia suspender goteo y dar tto
* Reiniciar goteo mas lento y/o diluido
 TRATAMIENTO COMPLICACIONES
Reacciones Anafilacticas: Suspender goteo antiveneno, manejar el ABC, O2 , Hidratacion y Medicación.

-Epinefrina IM 1:1000 (0,25 a 1 mg cada 5 a 15 minutos) o dopamina

IV 1:10000 (0,01 a 0,3 mg/kg) solo en shock franco, IV diluida 1mg/10ml o dopamina
-Dopamina 2,5 a 10-20 mg/kg en ancianos, cardiópatas o uso de betabloqueadores
-Corticoides (5mg/kg de hidrodortisona)
-Antihistaminico (1mg/kg de hidroxicina)
