Arrives duty on time, but when it comes to lectures and other meetings, Ms.
Cruz fails to comply.
She has a good nurse-patient relationship, performs her duty well but still
needs guidance in the decision making with regards to patient care.
As the medication nurse, I have seen her transition from having a difficult
time in adjusting with regards to tallying, computation, etc to a steady-nohassle way of managing the meds.
Arrives on time, usually early on time assigned. Is neatly groomed with
proper uniform. Has good interaction with patients and performs well on
duty.
Needs to focus on duty, he has the tendency to lose concentration if he is
given multi-tasks. But as compared from the last quarter, Mr. Santos has
improved well with regards to his responsibilities and his patient care.
Arrives duty on time, but when it comes to lectures and other meetings, Ms.
Cruz fails to comply.
Performs her duty well and has a good intrapersonal relationship with patient
and collegues.
Still needs to interact with patients more to gain their trust.

Nina Arrives on time and is willing to extend beyond working hours if needed. She
is becoming reliable when it comes to attendance, she could now meet the centers
standard for punctuality and attendance.
She has shown the ability to establish an excellent rapport with the patients and
staff. She constantly provided safe sympathetic nursing care to our patients.
She always presents herself in a cheerful manner to everyone in the center.
In the mid part of the quarter, I have noticed an increased number of absences
from Ms. Nina. Most of which are incurred due to flu and other health related
matters. She has been advised to avoid committing absences and has complied to
this on the latter part of the quarter.
Although Ms. Trinidad excels well in her performance, she still has a lot to learn
when it comes to cannulation. She still has to learn the different techniques and be
able to cannulate other patients in the center. She still has to be supervised. She
must also get better when it comes to assessment of patients problem and referral.
Ms. Sanchez arrives duty on time and is willing to extend beyond working hours if
necessary. She is now improving and becoming dependable when it comes to
attendance.
She is constant in establishing rapport and assistance with the patient and staff.
She never fails to bring a positive atmosphere to everyone.
