Definition
Hospital admission involves staying at a
hospital for at least one night or more
Purpose
- Provide registration services for patients
who will enter the treatment room
- Knowing the number of patients who
entered the treatment room

Procedure
1. Doctors advise patients to hospitalization.
2. Upon consent of the patient / family / person in charge of
the patient, the nurse IGD / POLY tell the receptionist that the
patient be hospitalized.
3. Nurses guide the family / patient responsible for registering
inpatients to the receptionist.
4. For patients who entered through the emergency
department, the receptionist asked the patient Medication Card
(for older patients) or record data / identity of patients with
complete (for new patients)

Admission to hospital
Description
An individual may be admitted to the hospital
for a positive experience, such as having a baby,
or because they are undergoing an elective surgery ,
or because they are being admitted through the emergency department.
Being admitted through the emergency department is the most stressful
of these circumstance because the event is unexpected
and may be a major life crisis.

Admission To Hospital
Before the person is taken to their
room, admitting procedures are
performed. The person's personal
data is recorded and entered into
the hospital's computer system.
This data may include :

name
Address home
and work telephone number
date of birth
place of employment
occupation

emergency contact
information, or the names and
telephone numbers of those
individuals the hospital
should contact if the person
being admitted needs
emergency care or their
condition worsens
significantly.
insurance coverage
reason for hospitalization
allergies to medications or
foods
religious preference,
including whether or not one
wishes a clergy member to
visit

Common for Patients


4.1. Receptionist offers inpatient services rates clearly to the patient.
4.2. If there is agreement from the family / person in charge of the patient, the
receptionist gave form "Statement of Payments" to the family / person in
charge of the patient to be filled and signed
4.3. Receptionist asked for assurances hospitalization to family / person in
charge of the patient in the form of ID / driver's license or other
identification
Once the form "Statement of Payments" filled in and signed by the patient,
give the form to the Medical Record to look for the file Status Inpatient
accordance with Number medical record and then Status Inpatient
delivered by officers of Medical Records to IGD / POLY destination

For Patients Using Insurance


Ask holdings owned by the patient's health insurance
When patients enter the working hours, ask the patient to take the
guarantee issued by the Company / Insurance related. When the patient
goes outside of working hours, the guarantee was taken the next day,
during working hours.
Asks sheet guarantees, photo copy of the insurance card, and a referral
from a health center (except for emergency cases) as a complementary bill.
Ask the patient to complete the other requirements relating to insurance
bills owned.
When the administration of the incomplete condition, family / person in
charge of the patients were given a maximum of 2x24 hours to meet the
requirements (for inpatients). If not met, the patient is considered PUBLIC.
Define and tell family / person in charge of the patient rooms that will be
occupied by a patient in accordance with a predetermined quota-related
insurance, by classifying Adult (Male / Female) and or the Son.

If the patient is asked to grade treatment (except


JAMKESMAS and JAMKESDA), give a "Statement of
Payment Willingness Difference Cost" to be filled and signed
by the patient / family.
Receptionist asked for assurances hospitalization to family /
person in charge of the patient (especially to patients who ask
for treatment to the next grade) form of ID / driver's license or
other identification
Once the form "Letter of Commitment Fee Payment
Difference" filled in and signed by family / person in charge of
the patient (patient requested special treatment to the next
grade), give the form to the Medical Record.

All inpatient administrative records that have been completed are given to
the medical record to look for the file status in accordance with the Patient
Medical Record Number and then Status Patient Medical Record delivered
by the clerk to the IGD / POLY destination.
Medical Records Officer recorded in the book of patient visits and
inpatient mark.
Receptionist informed to the hospitalization of the patient rooms that will
be used to prepare all the fittings and amenities.
The nurse prepares a new patient rooms.
After the inpatient unit is ready, the nurse told the receptionist that the
room was ready for occupancy.
Receptionist tells the nurse POLI / emergency room that has been prepared.
Nurse POLI / IGD bring patients to the inpatient room.
