SAFETY & LOSS PREVENTION PROGRAM SECTION - 1 SAFETY AND LOSS PREVENTION POLICY 1.1 COMPANY POLICY STATEMENT It is the aim and continuing effort of FIREFREEZE Est., to maintain an efficient and high standard of Safety and Loss Prevention Program, in order to avoid dist ress to persons and financial loss. This policy is maintained by providing a saf e wor ing environment, by adapting / fol1owing correct wor procedure and by usi ng the right and highly conditioned tools, plants and equipments. Pursuant to this policy, this manual is produced to outline the responsibilities of all the employees, i.e., from the supervision down to the labor pool level. All supervisory personnel shall do their best efforts to assure the safety of th eir wor force, and shall be responsible for the protection of all plants and equ ipments within the area of responsibility. All other personnel employed or agent s within the territorial responsibility shall ta e or cause to be imperatively t a en any measures in pursuance to the implementation of this policy.

1.2 HAZARD CONTROL PROGRAM The following principles and practices will be applied to preclude hazard. 1. 2. 1 A safe wor environment, plants and equipments will be provided while ex ecuting the wor to eliminate HAZARDS. 1.2.2 All personnel will adhere with the safety requirements of the job and wi ll refrain from executing any act that could endanger the life of other wor ers and/or damage to properties.

1.2.3 Sufficient qualified personnel will be provided to meet the job requirem ent. Excess personals Equipments and any obstructions will be removed from the w or site during the execution of the wor . 1.2.4 Closed Supervision and Chec -up should be done by a company safety offic er to all jobsites to ensure compliance of the safety rules and regulations impl emented.

1.2.5 Periodic meetings will be organized with the management and site impleme nters to see to it that programs are strictly followed and implemented. 1.2.6 Updated hazard control program list shall be distributed and properly di sseminated especially to all site implementers. ********** SECTION - 2 RESPONSIBILITIES FOR THE SAFETY PERSONNEL 2.1 THE MANAGER:

2.2. 1 Shall assume the overall responsibility for the active implementation of the Safety and Loss Prevention Program. 2.1.2 ty; Shall set a personal example, and to actively encourage and promote safe

2.1 .3 Shall ensure that all the employees, the management down to supervisor a re fully aware and conversant with the companys safety policies. 2.1.4 Shall ensure that all levels of supervision are qualified and receive ap propriate training on continuing basis, and to monitor the effectiveness of safe ty performances; 2.1.5 Shall reprimand any member in supervisory position, not discharging thei r responsibilities, and to personnel who abuse the use of equipment and/or commi tting any act that may endanger people or damage to company facility. 2. 1.6 Shall institute and promote action to prevent recurrence of any injury, accidents or damage to property, and to initiate analysis to monitor accident tr ends. 2. 1 .7 Shall appoint one safety officer for the enforcement of Safety and Loss Prevention Programs.

THE SUPERINTENDENT

2.2. 1 Shall understand and ensure that the Company policy is observed. 2 .2.2 Shall create safety awareness and ensure that all governing policy and s afety information are distributed, promulgated and actively implemented; 2.2.3 Shall recommend, plan and promote training for all levels of employee in order to assure that all personnel and supervisors under his area are competent to carry out and discharge their responsibilities;

2.2.4 Shall ma e himself available as a bac -up resource to the management and supervision, and to chec over wor methods and precautions with supervision pr ior to the start of wor s, and; 2.2.5 Shall set a personal example on site by wearing appropriate protective c lothing and equipments at all times. 2.2.6 Shall coordinate with the company safety officer regarding new safety ru les, regulations & program for adoption. 2 .2.7 Shall give warnings and penalties to all site engineer and supervisors a s to non compliance or misuse of authority and responsibility which cause damage , loss of life of employees and company properties.

THE SITE ENGINEER/SUPERVISOR

2 .3. 1 Shall be one of the persons to chec /master his site operation familiari ze and conduct an advance study of the wor site and environment; 2.3.2 Shall perceptibly identify possible wor hazard, unsafe condition, and a dminister plan/recommendation, in cooperation with the safety officer/engineer, for its implementation; 2.3.3 Shall organize sites to ensure that wor is being carried out with minim um ris to men, equipment and materials; 2.3 .4 Shall ma e himself and his supervision aware with the requirements of th e Company policy; 2.3.5 Shall be familiar with the wor permit procedures, and to arrange for qu alified person on site to receive wor permits;

2.5.2 Shall maintain a log record and see to it that each transport unit is pe riodically tested, inspected and maintained; 2.5.3 Shall conduct a periodic vehicles inspection and report defects to the w or shop for immediate action; 2.5.4 Shall ensure that all drivers are familiar with safe loading procedures, i.e., trailers maximum load capacity, securing and positioning of cargo; 2.5.5 Shall conduct wee ly, a safety driving rehearsal and ensure that all dri vers are familiar with the SAG traffic rules and regulation.

2.8.1 Shall advise management on all aspects of safety, i.e. Safety and Loss P revention Rules and Regulations, Codes and Practices, ways to improve existing w or methods, hazard prevention, current and new legislation and changes in safet y requirement in line with the company activity; 2.8.2 Shall coordinate and consult with the representative on all aspects of s afety and to report any possible hazards and corrective action to be ta en, whic h may be encountered on site; 2.8.3 Shall perform regular site survey to ensure that only safe wor procedur e are in operation and that health and safety requirements are being observed an d that, first aid facilities are adequate and in proper condition; 2 .8.4 Shall investigate and determine cause of personal injuries, other accide nts or dangerous occurrence and recommend ways of improvement to prevent recurr ence; 2.8.5 Shall maintain and eep full records of accidents, damages and injuries, and wor at assessing the accident trends and improve safely performances; 2.8.6 Shall advise employees of safe wor ing methods, circulate/promulgate saf ety information and stress that injury prevention and damage control are the int egral part of an efficient operation; 2.8.7 Shall ensure that necessary safety tools and equipment are provided and used at all times and ta e action of anyone not complying the safety rules with disciplinary rules;
