Pursuant to the provisions of Section 25.3 of the Implementing Rules and Regulations Part A of RA 9184 and in accordance with the provisions of Section 5 of Department Order No. 13, series of 1998, of the Department of Labor and Employment (DOLE), we hereby submit to you, as part of the Technical Proposal of our bid for the abovementioned contract, this Construction Health and Safety Program which, once approved by the Department of Health, shall be implemented by us for the said contract if it is awarded to us.

1.0 CONSTRUCTION SAFETY AND HEALTH COMMITTEE


Once CIRIACO CORPORATION has been awarded the above-mentioned project, it shall be mandatory on its part to organize the Construction Safety and Health Committee the earliest time possible and shall appoint members to head the said committee. It shall be of paramount interest of the company to hire qualified personnel that shall constitute members of the said committee. The Construction Safety and Health Committee shall oversee the implementation of the health and safety program that will be laid down in the process of constructing the above-mentioned project. The specific Health and Safety Program that will be followed are lined-up below. HEALTH AND SAFETY PROGRAM Introduction It is the companys objective to achieve the highest level of health, safety and environmental performance to ensure that the company is successful in the business. The company will conduct its operation in such a manner as to: Ensure a healthy and safe working environment for all workers involved in its operation Prevent all forms of accident that gives rise to loss of life, injury to personnel and loss or damage to property.

Encourage the raising of health, safety and environmental standards through training, use of approved plants, tools, and equipment. Treat safety and progress equally but safety will not be compromised.

Objectives Target Zero accident and eliminating lost time due to accident by eliminating unsafe acts or unsafe conditions. Accident Prevention Accident is very costly and causes human suffering and damage to properties All supervisors must accept the concept of accident prevention s part of their daily activities and the workmen must be convinced that the management is fully committed to safety. The first step is to reduce or minimize accident is to eliminate unsafe acts by people and unsafe condition of work place, plant or equipment. To provide proper guards to the exposed parts of machineries, install safety devices, maintaining good First Aid and fire-fighting equipment and conduct safety training course to staff. To provide safe working environment To promote safety awareness To observe and work with high safety standards To ensure safety and health of staff and sub-contractors employee on site To protect public from injury and damage to properties To ensure minimum or zero work disruption To provide training on safety at all levels

General Safety Rules Every employee has an obligation under his/her term of employment to prevent accident by: Utilizing the correct personal protective equipment (PPE) like wearing safety helmet and working shoes Employing safe working practices Utilizing correct working tools Keeping the job site safe and tidy. Ensuring other persons is not put at risks. Following the companys established safety regulations and procedures.

2.0 SPECIFIC SAFETY POLICIES


IDENTIFICATION OF RISK RELATED ITEMS First and foremost in the formulation of specific safety policies is the identification of risks related items that are present in the workplace. Construction industries are generally exposed to risks, some are inherent in the workplace and on the type of work to be performed while others are just simply the product of sheer carelessness and unmindful performance of the job on hand. HAZARD ELIMINATION After identifying the hazards, it is necessary to eliminate them immediately and correctly by formulating specific safety policies applicable to the risks present. It is only through this process that accidents that could result to injuries, loss of lives and damage or loss to properties could be minimized, if not totally eradicated. The specific safety policies would mostly deal on the following: 1. Identification of General Health, Safety and Environment (HSE) Rules and Regulations and the presentation of work-relevant reminders applicable in general. 2. Policies pertaining to the proper wearing of Personnel Protective Equipment (PPE). 3. Policies that deal on proper actuations in case of fire. 4. Policies that relate to the use of mechanical and hand tools. 5. Policies to be implemented when working with heavy equipment such as cranes and the likes. 6. Policies during excavation works and works under pits. 7. Policies related on working at heights. 8. Policies in the use of scaffoldings. 9. Policies in road safety 10. Policies while working on electrical units and the likes. 11. Policies on manual handling of equipment and materials 12. Policies on proper housekeeping 13. Policies on welding and cutting works 14. Policies on safe driving 15. And policies to avoid accidents and proper way to deal with it.

RESPONSIBILITIES A. THE PROJECT MANAGER The Project Manager who is the head at the site will be responsible for all activities and undertaking at site. In the same manner, he/ she shall also be responsible in the proper implementation of the safety and health policies that are formulated. He/ She should see to it that project safety plan/rules are ready for distribution to various sub-contractors and site supervisors. These should be handed-out in advance

for the workers to have ample time to read and prepare what they need in the performance of their duties. He/ She shall see to it that regular site meeting shall be held with the sub-contractor/ staff to ensure safety regulation is complied on site. He/ She shall ensure that all safety code and work procedures have been complied with and carried out by the site staff. Site Safety and Health Committee will be formed and regular meeting will be carried out. A ready copy of safety and health manual must be kept in the office for easy reference.

B. SAFETY OFFICER/ SUPERVISOR The role of Safety Officer / Supervisors at the construction site is to enforce and promote safety as set out in the project safety and health program. He shall be responsible in educating and instilling safety awareness to the workers. He shall give safety induction to all workers at site as well as to assist the subcontractors safety officer and to hold regular tool box meeting.

ACTION PLAN The action plan would be to hold safety review on a monthly basis, to hold discussions with site supervisors and operators prior to commence of regular activities, to conduct site safety inspections and to display safety posters/ signages at strategic locations in construction site.

3.0 PENALTIES AND SANCTIONS

PROJECT SAFETY POLICY The management of Ciriaco Corporation recognizes the importance of safety and health in making life more rewarding, and is committed to providing a workplace for our employees in which recognized hazard are controlled. It is the policy of Ciriaco Corporation to conduct their activities in such a way as to take foremost account of the health and safety of all employees and of other person in the construction site and to give proper regards to the conservation of the environment. To implement this policy, we not only comply with the requirements of the relevant legislation but also to promote in manner measures for the protection of health, safety and the environment for all that may be affected directly or indirectly by our activities.

The objectives behind this commitment are: 1. The safety and health of all CIRIACO EMPLOYEES is our first priority. 2. The only acceptable level of safety and health performance is one that prevents injury and accidents. 3. Safety and health are one responsibility that must be shared equally without exception by everyone within the organization, and 4. Supervision and management will be held accountable for the safety and health of the personnel for whom they are responsible. All employees will be required to make their safety and the safety of their fellow workers a first priority. As a condition of employment, each individual within the organization will be expected to conduct their daily task in a manner that is consistent with the philosophy and objectives in this policy.

SITE DISCIPLINE Personnel breaking the site safety rules and regulation, whether deliberately or not, shall be liable to disciplinary action not excluding dismissal from work as stated in your contractors handout.

4.0 TRAINING

Training Course All staff to attend relevant safety courses as and when required. Training course may be one of the following or the combination of the following: Safety courses First aid to injury Accident control techniques

SAFETY HANDOUT The safety practices described in this handout are for the protection of all personnel at the site. This handout, however, is not intended to cover all aspect of safety working throughout all operations. Prior to commencing work, all personnel will be given complete induction training in safe work practices relevant to their work area by their supervisor and/ or site officer. We would like to ensure that the physical working environment is established and maintained in a healthy and safe condition. No employee or contractor will be expected to carry out work that they reasonably consider being unsafe. This safety handout embodies almost every aspect of health and safety policies that the company wants to implement and contain basic rules and regulations on safety measures during construction. Each employee/ contractor will be held strictly accountable at all times for the knowledge and the observance of the Health, Safety and Environment (HSE) rules contained in this handout and covered in HSE site induction.

General Health, Safety and Environment (HSE) Rules and Regulations 1. No smoking, cooking or open flames are allowed except in designated area. 2. Firearms, deadly weapons, lighter and matches must be surrendered to the guard upon entering the project. 3. Firecrackers and pyrotechnic materials are strictly prohibited in the project. 4. The use of radios, walkman sets, cameras and other transistorized gadgets not intrinsically safe are prohibited except in designated areas. 5. ID must be worn visibly at all times when inside the project premises. 6. Entering/ walking within the project premises under the influence of liquor, prohibited drugs and other intoxicating substances is strictly prohibited.

7. Project staff, contract workers, foremen, supervisors and project workers must wear safety shoes, safety shirts, safety helmets and safety glasses upon entry and within the project premises. 8. Nobody is allowed to enter restricted areas of the project. 9. Parking shall be designated places only. 10. Pedestrian lanes and sidewalks shall be used when walking along roadways within the project. 11. Urinating at places not designated for the purpose is strictly prohibited. 12. Littering is prohibited. 13. Horseplay or creating unsafe acts is not allowed. In the same manner, initiating fighting or trouble is a safety hazard. Consequence shall be dismissal from site by offenders. 14. Sleeping and eating in construction areas can be dangerous and is not allowed. 15. Everyone must follow safety rules strictly.

D. PERSONNEL PROTECTIVE EQUIPMENT 1. In every worksite, the usage of PPE is a requirement. Every employee must use PPE provided by the employer. 2. Minimum personnel protective equipment a. Where there is danger of a falling object, safety helmet must be provided and used. 4. Tie the string of the helmet. 5. Repair damage of uniform. 6. Hearing protection is required in construction areas where noise level exceeds 85 decibels. 7. Dark glasses or life lines when working from high places. 8. Gloves shall be worn when engaged in activities where danger or hand injury exists. 9. Where there is water or wet areas, suitable waterproof boots must be provided and used. 10. Where there is corrosive or toxic substance, appropriate protective apparel equipment must be provided and used. 11. Where work is required in pits, underground chamber or enclosed areas where there is accumulation of toxic fumes or gases, suitable respirators or oxygen maskmust be provided and used.

SAFETY HINTS If you are in doubt, the safe or proper way to do the job is to ask the supervisor before starting it. Report any unsafe condition to your supervisor and stop unsafe actions immediately. Think before you act. Dont engage in horseplay or activities that distract others. Ensure that your work area is clean and tidy. Always wear your protective equipment. Dont take shortcuts, your safety and that of others are more important. Obey all safety rules and signs. Report all accidents, however small, and have them treated immediately.

FIRE Be prepared. Ensure that you know what to do in case of fire. Make certain you know your escape route and assembly point. Dont obstruct access to fire extinguisher. Learn to operate them. Do not smoke in forbidden areas. Raise the alarm if you see a fire.

MECHANICAL AND HAND TOOL Provide covers or guard on exposed gear and other moving parts. Always use the correct tools for the job. Keep hammerheads lightly wedged on their shafts. Never use equipment and tools that are damaged or excessively worked. Never make temporary repairs to equipment. Never use equipment that you are not trained to operate. Always wear the correct protective equipment for the job you are doing.

HEAVY EQUIPMENT CRANES Be alert when heavy equipment or machineries are in operation. Keep away from rotating area of the crane. Only the rigger can do signals to the crane operator. Stay away from the crane hoist. Only authorized person should operate equipments. Dont turn your back on moving machines. Dont dislocate safety signs and protective installations. Dont stand near delicate/ sensitive machine. Only authorized person should operate equipment. Inspect slings, chains and shackles, and check the minimum lifting capacity before use. Dont lift loads greater the rate capacity of your crane and lifting attachments. Use correct chains; slings and slinging procedures, making sure loads are secure. Do not allow loads to be suspended for external periods or left unattended. Use correct signals when operating crane or hoist. When moving any heavy equipment within confined areas, designate a signal man on the ground to guide you. Do not operate a defective or unsafe machine. All equipment fitted with movable attachments (buckets, blades, ripper, forks and augers) must be parked with attachments lowered to the ground. All bulldozer/ loader operator are responsible for checking areas beneath a work area to ensure that the material push down a slope or over an edge will not endanger employees or equipment.

EXCAVATION Watch out for deep excavation. Prevent soil from caving-in by providing adequate shoring. Always take precaution from the supervisor. Inform your supervisor when you notice something unsual on the excavation area. Consider the safety precaution when working underground to protect the soil from caving-in. Always use ladders to get in and out of excavation. Dont climb on the timber and never jump across. Before digging, make sure that the location of water, gas, electricity and telephone services is known and clearly marked. Keep soil heaps well away from the edge of excavations. Do not place materials, including tools, on or near the edges of excavation. Remember the people working below. Wear your hard hat and safety shoes at all times when working in an excavation. Dust masks, face masks and gas masks may be used as necessary. Remember to keep all access ways clear. Ensure that the walls of excavation are adequately shuttered to eliminate the risk of collapse.

13. Always fence off excavations and clearly indicate their existence with adequate sign and warning lights if left overnight. 14. To ensure public safety when doing excavation, work with proper signages. 15. Ensure areas to be excavated are stable and firm. 16. Machineries to be used for excavation should be in good operating condition.

K. WORKING AT HEIGHTS 1. Always use safety belt whenever a possibility of a fall exists. Check if safety belt is in good condition before use. Make sure safety belt rope is securely anchored. Dont depend on a man to hold you safety belt rope. Always anchor your safety belt rope to a fixed object. Never to a moving load or to equipment that may move. 2. Move carefully when you are working at high elevation. 3. Follow instructions and look at safety signboards. 4. Dont remove cross bracings hastily. 5. When passing under working area, give signal to co-workers and pass quickly. 6. Warning signs should be observed always. 7. Ensure that the platforms are properly secured with handrails and closely planked to prevent tripping and objects falling down. 8. Ensure that the area below you is clear of all unnecessary materials. 9. If safety nets can be employed in the work area, use them. 10. Ensure that the ladders are securely lashed at the top to prevent movement. 11. Scaffolding should be assembled properly. 12. Never use scaffolding ladder, which have not been inspected, tagged and passed for use. If in doubt, ask. 13. Ask assistance for a delicate job. 14. Coordinate with each other and discuss the method or flow of work. 15. All edges shall have proper barricade. 16. When works are being carried out at edges, safety belts must be used. 17. All opening shall be barricaded. 18. SIgnages shall be put up to warn the workers about the dangers or edges.

L. SCAFFOLDING 1. All scaffolds must be tagged prior to use. 2. Suitable handrails for personnel shall be installed on all scaffolds, guardrails and toe boards must be installed. 3. Scaffolds must be fully planked properly, supported and properly tied off.

4. Rolling scaffolds shall not be removed with someone on it. Remove or secure all tools and materials before moving wheels of rolling scaffolds. 5. Access ladder must be provided on scaffolds.

ROAD SAFETY Speeding and overtaking on the road is prohibited. Seat belts must be worn when vehicle is in motion. Never reverse your vehicle without assistance. Ensure your lights, brakes and horn are regularly serviced. Look around before crossing around. When walking around the project Dont use headlights at nighttime and whenever it is raining. Where works are being carried out in or along the public roads, the working area should be barricaded with sufficient warning signs or warning lights and flagman to direct traffic to slow down or direct them away from it. 9. For loading or unloading work, extra care should be taken by providing warning signs, lights and flagman.

ELECTRICITY Dont attempt to repair electrical equipment. Repair is an electricians job. Always check for defective cables, plugs or sockets. If found, do not use. Report them. Never overload electrical equipment. If a fuse blows, report it. Do not fit makeshifts. Dont let cables trail across the floor. Have your portable electric equipment inspected. Dont use lighting circuits for portable tools. Disconnect equipment when not in use, but dont pull the cable to disconnect, pull the plug. Keep all electrical equipment clean and dry. Dont use portable tools near flammable vapors or gases. Dont stand on a wet area when using electrical equipment. Never make temporary repairs to electrical cables, if damaged, replace. Work carefully at a high voltage area. Dont touch anything if you are not sure its safe to hold on. Dont stand near high voltage equipments. Execute signals carefully.

O. MANUAL HANDLING 1. Check the load to be lifted. If the load is too heavy, get assistance.

2. Lift the materials the correct way. 3. Always wear hand pails or gloves when handling sharp edge or rough materials. 4. Use caution when carrying long materials such as piping or ladders. Long materials must be carried from both ends. 5. Check the route you are going to take.

P. HOUSEKEEPING 1. Proper housekeeping is a first time impression of any construction sites. It is essential that housekeeping starts from the early stage of construction. 2. Dont leave the rubbish lying around, tidy up as you go. 3. Keep all gangways, aisles and stairways clear. 4. Wipe up spill oil, grease or liquid immediately. 5. Use metal containers for oily or greasy rags and waste. Always replace the lids. 6. Keep your tools safely when not in use. 7. Keep benches and worktops uncluttered. 8. Dont leave the loose tools on running machines. 9. Use signs whenever and wherever necessary. 10. Bags, containers of bundled materials must be stored in a manner that is stable and must have easy access to the materials. 11. Timbers should be stacked properly and at proper designated areas. 12. Ensure that no protruding nails are left unattended before stacking of timber is done. 13. Passageways and access roads shall be kept clear to provide for free and safe movement of material, handling equipment or workers. 14. Ensure all working areas are free from slippery condition.

Q. WELDING AND CUTTING 1. In all welding operations, leather gloves, aprons, spats and welding shields with appropriate lens must be worn as or when directed. 2. When welding or cutting is necessary in known hazardous or flammable areas, fire extinguisher must be readily available for immediate use if necessary. 3. Disposable cigarette lighters are not permitted to be used for lighting oxyacetylene torches, spark lighters are approved. 4. Oxygen and acetylene cylinders must be chained when in an upright position, Oxygen cylinder should be checked when lying down Acetylene cylinder must be stored, used and transported in an upright position only. 5. Care must be taken to keep welding hoses, cables or connections from being contaminated with oil and grease. 6. All equipment is to be maintained in a safe and serviceable condition.

7. Empty cylinders must be turned off, gauges removed, protective thread caps replaced and cylinders marked MT. 8. Be sure that you have adequate ventilation before doing any welding in a confined space. Oxygen gas should never be used for ventilation. 9. When welding or cutting above or near cables/ electrical equipment, welding blankets are to be used to prevent damage. 10. Where there is welding, cutting and grinding, suitable eye goggles or face shield must be provided and used.

SAFETY MEASURES ON DRIVING Dont disturb the person driving. Be sure to have license when driving. Observe the signal of the person who is conducting the signal. Be cautious on drivers without license. Keep safe speed. Dont use company cars for unofficial business.

S. IMPORTANT THINGS TO AVOID ACCIDENTS 1. Working area should be observed always. 2. Keep tools, equipment and materials on its proper place.

T. ACCIDENT In case of accident 1. Keep calm and approach with caution. Ensure that you or the helper is not endangered. Do not move the injured person unless there is a danger of further injury to him/ her. Leave lifting, loading and transportation of the injured to a qualified person such as nurse, unless it is absolutely necessary essential for you to undertake this yourself. If casualty has suffered electric shock and is still in contact with current, DO NOT TOUCH HIM/HER. Turn off the power if possible or separate him/her from source of electricity with some dry non-conductive materials. 2. Send (radio, emergency channel) someone to get the site, detailing your name, location, nature of accident and type and number of injuries. Apply first and to the best of your ability.

3. After an accident of a serious nature and do not move equipment until the site safety officer and construction manager inspects the scene, except when human safety is involved.

Reporting All accidents including near miss must be reported and investigated as early as possible to arrest its further recurrence.

5.0 WASTE DISPOSAL


On excavated materials, it shall be the task of the project manager to identify or designate areas suitable or needing filling materials. The project manager may coordinate with the local government executives to help him identify areas needing filling materials. Materials shall be dumped in such a manner that will not deface the surrounding of its aesthetic beauty and in such a manner that will not endanger the public or cause deterioration of the environment. Suitable equipment such as dump trucks, loader, roller and grader can be utilized to achieve this purpose. Scrap metals on the site will be collected on the day to day basis and collated in a separate container for proper disposal, as directed by the project manager on site. Used scaffolding materials, form plywood and form lumbers will be collected and protruding nails or wires will have to be removed. Forms and lumbers will be assessed if they can still be of worth in the succeeding works. If not, these materials will be kept in a suitable dry place for use later on in cooking preparation at the site. Dirty concrete that contain remnants of bentonite mix and initial grout during the concreting of bored piles are disposed of in bentonite bin. The mixture in the bin will later on be made to settle and the clear liquid disposed of in the bay. The solid particles that remained can be disposed of later together with other excavated materials that may still need to be disposed of later.

The Construction Safety and Health Program shall be executed and verified by the Contractors Project Manager and shall be submitted to the Bureau of Working Condition of the DOLE which may approve, disapprove of modify the same according to existing laws, rules and regulations and other issuance of the DOLE.
