TRIMMING CASTS Anatomy Teeth Alveolus Art Parallel and symmetrically trimmed bases Trimming objectives Aim: To produce

an accurate record of the teeth and oral anatomy Stable base with flush heels Symmetric base to facilitate evaluation of arch symmetry Esthetic adjunct for case presentations USE OF THE MODEL TRIMMER FOR TRIMMING CASTS: 1. Soak the stone cast. 2. Turn on the machine and the water supply (if it is not automatically connected to the on switch). Never trim dry! 3. Grasp the cast firmly and trim appropriately. Use caution for fingers, and beware of the blade grabbing the cast from your grip. 4. Periodically rinse the slurry from the cast. Otherwise, this will set into an additional mix of stone, and the cast will not be accurate. This may require rubbing under running water to remove. 5. When trimming is complete, allow the water in the trimmer to run until it is clean before cutting the machine off. 6. Note that the tray leading to the blade is removable, and should be periodically cleaned. MAXILLARY MODEL 1. Trim top of maxillary model parallel to occlusal plane 2. maxillary model height = 35 mm 3. Trim back of maxillary model perpendicular to median palatal raphe, stop short of hammular notch. NB. watch out for lower model posterior limit on class II patients. 4. Trim maxillary buccal segments a) Ideally 65 degrees to model back b) Alternatively, parallel to buccal alignment of the posterior teeth from the deepest extension of the muco-buccal fold at the molar to the canine eminence. 5. Trim maxillary anterior a) Ideally 25 degrees to the model back b) Alternatively, from the right and left canine eminence to the midline of the model. c) Trim maxillary posterior corners d) Ideally 75 degrees to maxillary anterior (12 mm)

e) Alternatively trim at right angles to a line connecting the maxillary canines and the posterior comer of the model MANDIBULAR MODEL 1. First, occlude the maxillary and mandibular model with the beeswax wax MI record Trim back of mandibular model flush with the back of the maxillary model 2. Trim excess plaster from the heels of the model and remove occlusal surface bubbles 3. Trim the wax record where it extends laterally into the vestibule or where it touches soft tissue. Invert models and trim back of mandibular model parallel to top of maxillary model 4. Trim bottom of mandibular model parallel to top of maxillary model a) Total height70 mm b) Mandibular model height = 35 mm 5. Trim sides of mandibular model a) Ideally 60 degrees to the back of the model b) Alternatively, parallel to buccal alignment of the posterior teeth at the deepest extension of the muco-buccal fold from the molar the canine eminence 6. Trim mandibular anterior in a smooth arc from the right canine eminence to the left canine eminence 7. Trim mandibular posterior corners a) Ideally 12 mm at 75 degrees to the maxillary anterior b) Alternatively, trim at right angles to a line connecting the mandibular canines and the posterior corner of the model 4. Finishing orthodontic models 1. Remove all bubbles and fill all defects/voids using a wax knife paintbrush with plaster and water 2. Sand the bases (art portion) to a smooth finish (400 fine). Maintain sharp line angles. 3. Thoroughly dry models (45 minutes minimum, preferably overnight) 4. Immerse in soap solution (Ivorytm Soap or Columbia Model Glosstm) for 30 minutes, rinse and polish with a damp cloth for final finish. 5. Model identification Type-written self-adhesive label on the back of each model with the following information: 1. Chart number 2. Patients name 3. Patients age Date of impression

EVALUATION (Lab 107) 1. Insure that all blebs are correctly removed from the occlusal surfaces of the teeth 2. Identify any voids and what in your technique probably caused them. CAUTION: WHEN TRIMMING CASTS, ALWAYS TRIM WITH THE CAST WET, AND FREQUENTLY RINSE THE SLURRY WATER/STONE FROM THE CAST!! 4. Follow the handout on trimming orthodontic casts. Using the compass, ruler and beeswax bite, trim the white casts to the appropriate dimensions. 5. Trim the buff casts for mounting to the depth of the facial vestibule. Do not flatten the base unless it is excessively thick. If the base is ground smooth, roughen with a laboratory knife prior to mounting on the articulator. 6. Determine how many of the following you can locate on your casts: vestibule Hamular notch frenum fovea palatine attached gingiva tuberosity retromolar pad junction of the hard and soft palates retromylohyoid fossae

SEQUENCE FOR TRIMMING THE UNMOUNTED, ORTHODONTIC DIAGNOSTIC STUDY CASTS

1. Place the maxillary cast on a flat surface and visually determine which area of the top of the base to grind to make it parallel to the occlusal plane and the bench top. (Fig. 1) Then trim the cast as shown to approximately 35mm thickness from the top of the cast to the occlusal plane. 2. The back of the maxillary base is trimmed perpendicular to the median palatal raphe. This back border of the cast should be trimmed to the hamular notch area. (Fig 2) 3. Next trim the right and left sides of the maxillary cast so they are parallel to the buccal alignment of the posterior teeth (Fig. 3) and trimmed to the deepest extension of the muco-buccal fold. (Fig 4) 4. The anterior border of the maxillary cast is now trimmed from the center of the cuspid area to a point bisecting the midline of the cast on both the right and left sides. (Fig. 5) 5. The posterior corners are then trimmed at right angles to a line connecting the maxillary cuspids and the posterior corners of the casts. (Fig. 6) 6. Occlude the maxillary and mandibular casts with the wax centric occlusion record. If the wax record touches any soft tissue areas on the casts, these areas of the wax record must be trimmed away. Also, if any land area of the casts interfere, they must be reduced. 7. With the maxillary and mandibular casts occluded in the wax record and the casts inverted the posterior border of the mandibular cast is trimmed until it is flush with previously established posterior border of the maxillary cast. (Fig. 7) 8. Then the base of the mandibular cast is trimmed so that the top of the maxillary cast, the occlusal plane and the bottom of the mandibular cast are all parallel. The total cast height should be approximately 70mm. (Fig. 8) 9. The sides of the mandibular cast are trimmed so they are parallel to the buccal alignment of the posterior teeth. (Fig. 9) 10. The anterior area is trimmed in an arc from the right cuspid area to the left cuspid area. (Fig. 1 0)

11. The posterior corners of the mandibular cast are trimmed at right angles to a line connecting the mandibular cuspids and the posterior corner of the casts (Fig. I 1) Remove and/or repair any defects or artifacts on the casts.
