An Explanation of Payment Plans and Insurance Policies
We would like to first express our appreciation for your
referrals and continued confidence in our services. Also, we wanted to take this opportunity to give you information on insurance companies which we are preferred providers, as well as services and financial options that are available to your patients. We are providers for: MetLife, Delta, Ameritas, Guardian All indemnity or PPO plans may be used in our office based on our UCR or out-of-network fees. It has been our experience that our fees fall within or relatively close to many of those fee schedules listed. We will gladly call on insurance benefits and eligibility in order to determine as accurate estimated patient portion as possible. Our professional team will prepare and file claims on behalf or your patients. We do ask that your patients be prepared to pay their estimated portion after insurance consideration at the time of their appointment. For patients convenience we accept Visa, MasterCard, Discover and American Express, as well as ATM debits. For patients who would like further options we offer third party financing with Care Credit, its convenient and takes only five minutes to process. Procedures
We provide surgical and nonsurgical endodontic treatment. When asked, we will complete post and core build-ups to ensure the patient returns ready for restorative treatment. We review each patients personal treatment needs prior to care with emphasis on the value to restore the tooth for full function.
