ACTIVITIES OF DAILY LIVING

ADL 1. Nutrition

Before Hospitalization The clients choice of food is mostly hotdog with rice. He eats three times a day, usually 1-2 cups of rice, hotdog or tocino and drinks 1-2 glasses of water. The father of the patient stated that he drinks at least 6-8 glasses of water everyday.

During Hospitalization The client stated that he started his hemodialysis he now only eats a cup of rice with any food prepared by their kasambahay. He still eats a heavy breakfast everyday. During lunch and dinner, a variety of pork and fish with vegetables are often served to him with 1 cup of rice. The client stated that he drinks a bottle of propel every meal.

Preferences in the tastes, smells, flavors, temperatures, colors, shapes and sizes of food influence the peoples food choices. (Kozier et al., Fundamentals of Nursing., 8th ed., vol.2 p. 1238) An adequate diet for an adolescent is at least 6 11 servings per day as well as 2 -3 serving of meat, 3 -5 serving of vegetables and fruit and 8-10 glasses of water a day(8801100mL).(Fundamentals of Nursing 5th edition by Taylor page 1246) Interpretation:

The client stated urinates 2-3 times a day. He described his urine as light yellow. According to him, he does not hold his urination. However, he verbalized that he has observed that his urine seems to be only in small amount. On the other hand, when it comes to defecation, he said that he usually defecates once a day. And he described his feces as semi-formed. He stated that does not feel pain in defecation.

The client decides to change his diet, because of his condition. The client is not usually exercising but he is usually in their house playing with his dog and sometimes do some household chores. His typical leisure activity is watching TV.

The client has the same activities as before hospitalization but sometimes he just want to rest because he becomes tired easily now.

4. Hygiene

The client stated that he takes a bath everyday and brushes his teeth twice a day. He often washes his hand especially before eating.

Analysis: Substance abuse is a major threat to the health of young adults. Alcohol, marijuana, amphetamines, and cocaine, prolonged use can lead to physical and physiologic dependency and subsequent health problems. Addiction, or physical and psychologic dependence on a substance, is related to properties of the substance, the individual user, and the social network of the individual ( Fundamentals of Nursing, Kozier,et al, 8th ed,

5. Substance Abuse

The client said that he began smoking when he was in elementary. During that time he usually consumes 1-2 pack/day. He kept on smoking for about up to now. And he started drinking when he was also in highschool. He said that he drinks occasionally usually 3-4 bottles. He also stated that hits marijuana since he is highschool up to now. According to him, he has tried these because of peer pressure.

The client stated that the client still smokes but stopped drinking alcohol. He also stated that he does not stop using marijuana.

Analysis: Sleep quality is a subjective characteristic and is often determined by whether a person wakes up feeling energetic or not. (Kozier, et al. 2008. Fundamentals of Nursing, 8th Ed. Pearson Prentice Hall, New Jersey, Vol. 2, pp. 1172) Most healthy adults need 7 9 hours of sleep a night (National Sleep Foundation,n.d.b.). However, there is individual variation as some adults may be able to function well with 6 hours of sleep and others may need 10 hours to function optimally. 6. Sleep and Rest

The client sleeps 6-7 hours a day and takes a nap in the afternoon (maximum of 1 hour). His said that his sleeping environment is okay but he can be easily interrupted by noise. He said he can get to sleep fast but also easy to be awaken.

The client stated that his sleeps 5-6 hours a day and usually talks while sleeping. He can be easily interrupted but he said he has no difficulty in sleeping.

The data gathered shows that the client lacks of sleep and easily awaken by noises in the environment.
