c) Proceso Quirúrgico (Describir los pasos principales de la técnica médico-quirúrgica con el instrumental a usar).

* Compresses and gauze are counted.

* Incision is made in the xiphoid apophysis.
* Hemostasis of the tcs with electrosurgical scalpel and separates with richardson separators.
* The pericardium is searched with dissecting forceps and allix forceps, when the surgeon is already in the pericardium, the
window is made and all the tamponade is suctioned with yankawer cannula.
* In this surgery it was necessary to perform sternotomy due to perforation of the heart.
* The incision was made with a 15 blade scalpel and the sternotomy with an oscillating saw.
* The stitches were removed and the sternal separator was placed for visualization.
* The suction is continued for better visualization of the surgical field and with prolene 4/0 the suture of the perforated part is
performed.
* The patient is stabilized and the sutures are checked.
*The chest tube is placed in the anterior incision with 2/0 polyester.
*Chest tube is placed and secured with polyester.
*The compresses and gauze are counted.
*A clean compress is placed as protection to perform the sternal sutures with surgical wire 6 and the compress is removed.
* The tcs are sutured with Poliglactin 0 and the xiphoid apophysis is sutured.
* The skin is sutured with monocryl 3/0.
* Apply dermabond sealant on the wound, wait for it to dry and place the healing dressing.
* Make a trouser and stop the chest tube and place Tegaderm.
