ARTIST CODE OF CONDUCT AGREEMENT

Please read the following articles carefully. When you’re finished, sign and date at the bottom of this page
to indicate your understanding and agreement.

• I understand that this is a family event. In light of this, I understand that all music must be acceptable
family entertainment. No profanity, crude or sexual lyrics, violent lyrics, racial/ethnical/sexist or other dis-
criminatory lyrics will be tolerated.

• I understand that any merchandise that the band/artist I represent may choose to sell in the merchan-
dise area must be approved by the Ohio Rock for Relief artist selection committee to ensure that it is ac-
ceptable to be presented in this atmosphere.

• I understand that this is a charitable event. Although the band/artist I represent will not receive any di-
rect compensation for performing, I understand that the money from the merchandise we sell is ours to
keep. However, I agree not to solicit or accept any other form of compensation, either monetary or other
tangible donation, from the guests of the event, other artists, sponsors, etc.

• I understand that the decisions of the Ohio Rock for Relief artist selection committee are final. Should I
have a grievance with a decision or ruling by the committee, I understand that they will consider all com-
ments or questions, but I agree to abide by their final decision in all matters.

• I understand that no alcohol or illegal drugs will be permitted on the event grounds at any time, nor will
the consumption or use of any of these substances be tolerated in any way during the event. I also under-
stand that the members of, representatives of, or guests of the band/artist I represent will not be admit-
ted to the grounds while under the influence of alcohol or illegal drugs, and will be asked to leave if it is
suspected that they are in violation.

• I understand that there will be a designated artist area for smoking, and that any tobacco products must
be kept to these areas.

• I understand that any material (including posters, fliers, graphics, web sites, etc.) that I use, either on-
line or in print, to promote this event in any way must be submitted to, and approved by, the Ohio Rock
for Relief committee. I also understand that I am free to use any graphics provided by the Ohio Rock for
Relief to promote the event.

• I understand that the materials I include with this packet (CD, picture, bio, etc.) will not be returned to
me.

By signing below, I indicate that I understand and agree to the aforementioned terms and conditions. I also under-
stand that this agreement can be modified at any time. I understand that, should this agreement be modified, I will be
notified of the revisions at the contact information I provided on the application and will be required to sign and sub-
mit acknowledgement of my understanding and agreement to the revised terms. I understand that any violation of
these terms can result in repercussions as severe as immediate removal of the band/artist I represent from the artist
roster and the event grounds.
