The Ladies Who Laugh House Party Video Contest (the Contest) is subject in all respects to the complete Official Rules available at the Ladies Who Laugh House Party website at www.houseparty.com/ladieswholaugh at the Contest section of the website or by sending a self-addressed postage-stamped envelope to Ladies Who Laugh House Party Video Contest Rules Request, c/o House Party, Inc., One Bridge Street, Suite 105, Irvington, NY 10533 (Vermont residents may omit return postage). THIS AFFIDAVIT AND RELEASE MUST BE SIGNED ON PAGE 2 BY ALL PARTICIPANTS IN THE VIDEO ENTRY (INCLUDING THE SUBMITTING PARTICIPANT). ALL PARTICIPANTS IN THE VIDEO MUST BE AT LEAST 18 YEARS OF AGE. THIS AFFIDAVIT AND RELEASE MUST BE RETURNED TO HOUSE PARTY, INC. (SPONSOR) BY THE TIME AND IN THE MANNER AS PROVIDED IN SECTION 4(B) OF THE OFFICIAL RULES. This Affidavit and Release is a legal agreement between each Video Entry participant and Sponsor regarding the Video Entry and participation in the Contest. Affidavit and Release.

a. I am a legal resident of one of the fifty (50) United States of America or the District of Columbia and at least eighteen (18) years of age . I am not an excluded participant as provided in the Official Rules. b. I am submitting this Affidavit and Release with the understanding that it will be relied upon to determine my eligibility to participate in and receive any prize in the Contest. c. I represent that I have read, understood and complied with, and will continue to comply with, all of the rules and regulations of the Contest, including, without limitation, the Official Rules; all of which rules and regulations shall be binding on me. I further represent that I have not perpetrated (and will not perpetrate) any fraud or deception in entering or participating in the Contest, or in claiming any prize that may be awarded to me. I HEREBY AGREE TO BE BOUND BY ALL OF THE TERMS AND CONDITIONS OF THE OFFICIAL RULES, WITHOUT LIMITATION OR QUALIFICATION. d. I hereby irrevocably, fully and unconditionally agree to be bound by and accept the Official Rules, including, without limitation, the grant of rights set forth in Section 4(d) of the Official Rules (How to Enter; Submission of Entry Materials; Video Entry Requirements), and the making of the publicity release set forth in Section 8 of the Official Rules (Publicity). e. By participating in the Contest, I hereby forever and irrevocably RELEASE, DISCHARGE, INDEMNIFY AND HOLD HARMLESS, Sponsor and its parents, subsidiaries, divisions and affiliates; any and all persons and entities responsible for sponsoring, fulfilling, administering, advertising or promoting the Contest or supplying any prize (including, without limitation, Kimberly-Clark Corporation), and the employees, officers, directors, owners, representatives, agents, heirs, successors, assigns and insurers of each of them (collectively, the Releasees) from and against all losses, claims, damages, liabilities, costs or expenses (collectively, Losses), arising in whole or in part, directly or indirectly, from my entry and/or participation in the Contest, and/or my receipt or use of any prize awarded in the Contest. The foregoing release includes, without limitation, any and all LIABILITY, BODILY INJURY, DEATH, LOSS OR DAMAGE to me or any third party arising in whole or in part, directly or indirectly, from any prize or the use of any prize, the award, acceptance, possession, use or misuse of any prize, and entry and participation in the Contest. I further COVENANT NOT TO SUE any of the Releasees with respect to any matter addressed by the foregoing release, or encourage or assist any third party in doing so. This is a complete RELEASE and DISCHARGE of all my claims and rights against the Releasees and no action will be taken by or on behalf of me with respect to any such claims and rights, it being understood that the Official Rules and this Affidavit and Release, shall be binding upon me and my successors, assignees, subrogors, insurers, heirs, next of kin, executors, representatives, and legal and personal representatives. I UNDERSTAND AND AGREE THAT ALL RIGHTS UNDER SECTION 1542 OF THE CIVIL CODE OF CALIFORNIA AND ANY SIMILAR LAW OF ANY STATE OR TERRITORY OF THE UNITED STATES ARE HEREBY EXPRESSLY WAIVED BY HIM/HER. SECTION 1542 READS AS FOLLOWS: CERTAIN CLAIMS NOT AFFECTED BY A GENERAL RELEASE. A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS WHICH THE CREDITOR DOES NOT KNOW OR SUSPECT TO EXIST IN HIS OR HER FAVOR AT THE TIME OF EXECUTING THE RELEASE, WHICH IF KNOWN BY HIM OR HER MUST HAVE MATERIALLY AFFECTED HIS OR HER SETTLEMENT WITH THE DEBTOR. f. I will not make any attempt to rescind, limit or disaffirm this Affidavit/Release, or any portion hereof.

g. The Official Rules and this Affidavit/Release shall be binding upon me, and my successors, assignees, subrogors, insurers, heirs, next of kin, executors, representatives, and legal and personal representatives.
