PROVISIONAL OFFER OF ADMISSION

After due consideration of your application for admission into this polytechnic, I am pleased to write on behalf of the polytechnic to offer you provisional admission to undergo HND Business Administration [Evening] Course in the School of Management Studies in the 2010/2011 academic year beginning 29th December, 2010 on the condition that: (i) (ii) All particulars provided on your Application Form are correct in every respect; You will present at the time of registration the originals of credentials listed in your application form for scrutiny. G.C.E./WASC/SSCE Examination result slips will be presented, in addition to the proper Certificates. If the proper certificates have not been issued, the result slips alone will be accepted provisionally. If at any time, during the course of yout study, it is discovered that you do not satisfy the minimum entry requirements prescribed for your course of study at the time of this offer , or that any of the qualifications you purported to have had is false or incorrect, you will be withdrawn from the Polytechnic forthwith. If any other information given by you on your application form is found to be false or incorrect; your admission will be withdrawn.

The name by which you are hereby admitted and by which you will be registered is the one which will appear on any certificate that the Polytechnic may issue to you on successful completion of your course except change of name is by sworn affidavit or marriage certificate. The offer is not transferable to another session

(vi) You will sign a declaration of good behaviour on your arrival at the Polytechnic.

2. As you have claimed to be an indigene of Kogi State, you are required to present a certificate to this effect from the Chairman of your Local Government Area. 3. You are hereby informed that all fees must be paid at the beginning of the session. The current rate of fees can be obtained from our website. 4. Due to shortage of accommodation, students shall be given accommodation on first come , first serve basis 5. You are requested to produce medical Certificate of Fitness from the Polytechnic Clinic during registration
