Patient Instructions for COVID-19 Testing

1. Arrive at the testing site at scheduled time.

2. Bring proof of Residence (Utility Bill/State Issued ID).

3. PLEASE SHOW THE “APPROVED FOR TESTING” SIGN ON YOUR PHONE THROUGH YOUR WINDSHIELD ON ARRIVAL.

4. Do not roll down your windows unless instructed by a staff member at the testing site.

5. AT NO POINT YOU ARE ALLOWED TO EXIT YOUR VEHICLE.

6. Valley Med Urgent Care will contact you as soon as results are ready.

IMMEDIATELY AFTER THE TEST YOU ARE OFFICIALLY IN QUARANTINE; THIS WILL SERVE AS YOUR WORK DOCUMENTATION UNTIL WE FORMALLY
CONSULT YOU WITH THE RESULTS. RESULTS WILL BE READY IN 4-5 BUSINESS DAYS.

2. Traiga comprobante de residencia (factura de servicios públicos / identificación emitida por el estado).

3. AL LLEGAR POR FAVOR MUESTRE EL SIGNO "APROBADO PARA LA PRUEBA" EN SU TELÉFONO A TRAVÉS DE SU PARABRISAS.

4. No baje las ventanas a menos que se lo indique un miembro del personal en el sitio de prueba.

5. EN NINGÚN PUNTO SE LE PERMITE SALIR DE SU VEHÍCULO.

6. Valley Med Urgent Care se comunicará con usted con los resultados tan pronto como estén disponibles.

POR FAVOR SEGUIR INSTRUCCIONES

1. COLLECT: Insert the swab 0.5-1 inch into the RIGHT one of
the anterior nares. Rotate the swab 3 times against the nasal
mucosa for about 15 seconds and withdraw. Repeat with the other
LEFT anterior nares using the same swab. Do not let the swab
touch any surface before placing it into the collection tube
