CHECKLIST ON TURNING AND REPOSITIONING A CLIENT ON BED

Assessment
1. Asses client’s physical abilities, level of
comfort and ability to follow instructions.
2. Check client’s chart for diagnosis
Planning
3. Wash hands.
4. Organize and check the device for its
function.
Implementation
5. Identify the client. Explain the procedure to
the client.
6. Raise bed to high position
7. Put the bed in a flat position
8. Secure all tubes, drains and IV’s.
9. Place or assist client in a starting position.

Moving client from supine to side lying position


10. Have the client flex his knees and place his
arms across his chest
11. Place your hands on the client’s shoulders
and hip on the far side.
12. Spread your feet, flex your knees and place
on foot behind the other, while rocking
backward, gently roll the client towards you.
13. Raise the bed side rails and move to the
opposite side
14. Adjust the client until he/she is in correct
side lying position.
15. A pillow may be placed to support client to
head and arms.
Turning client from supine to prone position
16. Move the patient to the extreme edge of
the bed
17. Raise the side rail on that side and move to
the other side of the bed.
18. Place the client’s arm under his buttocks
with the palm up, bringing his far leg over
the leg near you.
19. Turn client face away from you. Grasp the
client’s far hand with your hand and his far
hip with you other hand.
20. Spread your feet, flex your knees. Place one
foot behind the other and gently pull and
roll the client towards you unto his
abdomen.
21. Move the client into the center of the bed.
22. A shallow pillow may be used to support the
client’s head.
Turning client from prone to supine position
23. Place the client’s far hand under his far
thigh. Cross his near leg over his far leg and
turn the client’s ace towards you.
24. Reach under that client;s hips to grasp far
hand. Place your second hand on his bear
hip and spread your feet and flex your
knees.
25. Pull the client’s hands while pushing on his
hip, gently roll his body away from you unto
his back.
26. After the move is completed, a footboard
may be used to support the client’s head,
arm and lower back.
27. Move bed side table near to bed and place
items frequently used within reach of the
client.
28. Wash hands
Evaluation
29. Evaluate client’s tolerance and response to
the procedure.
30. Evaluate the client’s position for alignment.
31. Evaluate any abnormal findings on skin
assessment.
Documentation
32. Record the client’s position
33. Record any complains of pains, symptoms
and discomfort observed during the
repositioning.
34. Record the client’s response after the
procedure.
