With the change in office staff, CPR / TB cannot expire within the nursing semester.

Considering this, students will be required to take the CPR class / TB either in late
December or June / July. The preferred time is June / July since availability of CPR
classes in late December may be limited because of the holidays and the CPR card will
cover both semesters.

CPR / TB status will be verified by the clinical instructor the 1st day of clinical.
Remember: CPR proficiency / recertification and TB test is required yearly.

Prior to or on the first day of clinical, student are to submit evidence of completion of the
above requirement. This includes:
 Proof of CPR Currency as documented by
o Visual inspection of original CPR card signed by the student,
o Copy of CPR card (includes both sides of the card) and
 Proof of Negative Tuberculin Status as documented by EITHER:
o Photocopy of negative reading of tuberculin skin test done
o if your skin test results have been positive in the past,
1. Negative chest x-ray &
2. System review performed by Student Health Center

The responsibility of maintaining currency of these requirements is the student’s.

Students will not be allowed to progress in the semester


 Student does not provide “proof” of the above prior to or on the first day of
clinical
 CPR card /TB expires within the semester time frame.
This will require students to submit a request for reentry into the nursing program being
aware that reentry is not guarantee and is on a space available basis.

The following form will be completed, given to the student to present to their clinical
instructor to assure entrance into the course.

This sheet must be completed by your clinical instructor prior to or on the first day of clinical. You will not be
allowed to go to the hospital / clinic if it is not completed and given to your instructor. Please help all of us get
the new semester off to a good start without clinical absences.
• CPR Currency: It must be valid for the entire semester. Visual Inspection of signed CPR card
is required + submit a copy of the card
 Negative Tuberculin Status as documented by EITHER:
1. Photocopy of negative reading of tuberculin skin test done
2. Or, if your skin test results have been positive in the past, negative chest x-ray and yearly
physical system review completed by the Student Health Center
