Activities in the Teacher Appraisal System

ACTIVITIES Proposed Schedule Persons Involved


I. Preparatory Appraisal Activities
1. The teacher/master teacher and the May to June Teacher School Head/
school head jointly review the TSNA Representative

II. Performance Appraisal Proper

A. For Formative Appraisal June to October School Heads/Instructional Leaders
ACTIVITIES
1. Teacher/Master Teacher prepares the

evidences of the professional throughout the year
development progress of learners, school
outcomes to verify performance.
2. The teacher completes CB-PAST Form 1
and the master teacher completes the October Teachers/Master Teacher
CB-PAST Form 2 as self assessment.
3. The school head verifies and validates the
teacher/master teacher self- assessment School Head
with the use of Form 1 and 2.
4. The instructional supervisors (school
heads, education supervisors, district July to October Teacher,
supervisors, department heads) make Master Teacher,
actual observations of the teacher Instructional Supervisors/Leaders
performance in the classroom/school
setting. A pre- and post observation
conference are required.
5. Teacher/Master Teacher and school
head/instructional leader hold October Teacher,
conference on the individual formative Master Teacher,
appraisal result and plans for instructional School Heads/Instructional Leaders
support for professional development in
critical areas.
6. For instructional Supervision to support
the Teacher Form 3/CB-PAST Form 3 Whole year round Teacher, Instructional
shall be used by instructional leaders Supervisors/Leaders
during their supervisory visits year round.
Teachers/ Master Teachers,
B. For Summative Appraisal Activities November to March Instructional Leaders School Head
Learners
1. Teacher/Master Teacher prepares the
Portfolio to house the materials and evidences Whole year round Teacher/ Master Teacher
of the professional development progress of
learners, school outcomes and to verify
performance.
2. The Teacher completes CB-PAST Form 1 and
the master teacher completes the CB-PAST February Master Teacher
Form 2 for summative appraisal to include the
Plus Factor.
3. The school heads verifies and validates the
Teacher/Master teacher self-assessment with February School Heads
the use of Form 1 and Form 2.
4. The school instructional leaders (department Teachers/Master Teacher
chair, coordinators, education supervisors, Starts November Teachers
district supervisors) utilize CB-PAST Form 3- Master Teachers
A, 3-B, 3-C to make actual observations. A Ends February School Heads
conference will be held immediately after Instructional leaders
observation.
5. School head and teacher/master teacher hold Teachers
conference and complete final summative March Master Teachers
evaluation. School Heads

C. Post Appraisal Activities


1. Final submission and approval of the Teacher Teacher
