AFFIDAVIT OF DISCREPANCY

I, GRACEL JANE SALERING PLAZOS, of legal age, single, Filipino and a resident of
________________________ after having been duly sworn to in accordance with law, hereby depose
and say that:

1. I am the legitimate daughter of the Spouses _______________ and __________________, born

on 24 July 1997 in Cagayan de Oro City;

2. I applied for Teacher’s Licensure examination with the Professional Regulatory Commission on
_________. In the said application, my date of birth was inadvertently indicated as 24 July 2017.
A copy of the said application indicating such date is herein attached and made an integral part
hereof as Annex “A”

3. Said error or discrepancy in my application must have been caused by mistake or inadvertence;

4. One of the requirements for the application of the licensure examination is a certified true copy
of my birth certificate from the Philippine Statistics Authority (PSA);

5. Pursuant to such requirement, I requested for a copy of my birth certificate with the PSA, City of
Cagayan de Oro and my date of birth is written as 24 July 1997, which is my true and correct birth
date. A copy of said birth certificate is attached hereto as Annex “B”;

6. I have executed this affidavit to attest to the truth of the foregoing, and to serve as a supporting
document, to maintain that my true and correct date of birth is 24 July 1997 instead of 24 July
2017, in accordance with the Certificate of Live Birth issued by the PSA, and for any legal purposes
it may serve.
