3. To provide seamless and hassle free sewices to Haryana Govt. Empioyee/Pensioners and
their dependents, the empanelled private hospitals shall extend all normal courtesies and
facilitate the admission, treatment and &scharge of Haryana Govt. beneficiaries with no
delay or denial of medical facility.

4. A Nodal Officer shall be appointed at 1eve1 of empanelled private hospital. The Nodal will
communicate with the concerned Civil Surgeon or/ and department in case of any
srievance.

5. In case oI any cmerge ncy/disasters /overfiow oI parients in Govt. Hospiral durjng

epidemics the empanelled hospital will agree co share their ambulances, moruary ,
ICU/CCU . burn unit. ward bcds erc. in need based manner

6. All starutory Rules/Guidelines/Acts/Notflcations issued by State Govt. from time to time

(As already mentioned in the application form for New Empanelment Policy) shall be
followed by the empanelled hospitals. The same shall be appllcable on existing private
empanelled hospitals.

7. The hospital will abide to all the National Programmes and.its related IEC activities. The
data needed for National Program or Health In&cators must be shared with Health
Deparcment. The same shall be applicable on existing private empanelled hospitals.

8. The hospital shall follow the birth 6g death guidelines issued by the State Government
from time to time. Any software developed by the Govt. wi,ll have to be adopted by the
hospital.

9. Ir will be rhe responsibility of the hospital to provide accre&tation certificate and further
renewal certificate well bclorc the expiry date.

10. Guidelines already framed under State Govt. policy dated 21.05.2015 for empanelled
hospitals shall be applicable on new empanelled-hospitals. However, when New Packages
are made {g or some services ale started in Cashless mode new zuidelines shall be issued
lor compliance with consenr of concerned hospiral.

t1. The above listed Full NABH hospitals will remain empanelled for period of three years
subject to validity of accre&tacion ce ficate. Thereafter, the hospital will appiy afresh.
The above Iisted Encry Level NABH Hospitals will remain empanelled for a period of one
year subject to validity of accreditation certiJicate. The Entry Level NABH Hospitals will
have to get fu1l NABH accreditatlon within one year of erhpanelment failing which these
hospitals will automatically stand de-empanelled without giving any prior notice.

Health Department reserves the right to call for record of treatment of any patient from
any empanelled hospital to ensure no mal-practise takes place.

2. In continuation of the Health Department letter No. 2/56/2018-IHBIII dated 20.03.2018,

the Government has declded to continue the empanelment of . Rajasthan Medical Centre,
Chandigarh Road, Tohana, Fatehabad and reimbursement rate of medical expendirure
incurred for treatment on indoor basis by the Haryana Government employees/pensioners and

3. However, the remaining guidelines of the New Empanelment policy and relmbursement
sha1l remain unchanged.

4. These instructions will be applicable from 31.07.2018 to 15.04.2019, instructions may
