Resumen
La salud en Colombia es ahora un derecho fundamental que tiene que ser provisto y protegido
por el Estado. A partir de metodologías de análisis de sistemas de salud propuestos por la

OMS y el Banco Mundial, se evidencian las falencias, fortalezas y dificultades del sistema de
salud con respecto a la ley estatutaria aprobada en febrero de 2015. Éstas incluyen la
fragmentación y especialización de los servicios, barreras de acceso, incentivos no alineados
con la calidad, débil gobernanza, múltiples actores con poca coordinación y sistema de
información que no mide resultados. Es necesario un acuerdo social, un equilibrio y control
de la tensión por parte del Estado entre el beneficio particular y el beneficio colectivo.
El Sistema General de Seguridad Social en Salud colombiano (SGSSS) es central para el
desarrollo social y económico del país. Creado mediante la Ley 100 de 1993, este sistema
rompió la historia de Colombia en un “antes” y un “después”. Se enmarcó en los principios de
universalidad, cobertura, integralidad, solidaridad, igualdad, suficiencia, participación y
unidad. Reemplazó al Sistema Nacional de Salud que funcionaba desde 1975, creó el sistema
de aseguramiento y estableció relaciones de mercado entre las aseguradoras denominadas
“empresas promotoras de salud (EPS)” y las “instituciones prestadoras de servicios de salud
(IPS)”.

Abstract
Health in Colombia is now a fundamental right that must be provided and protected by the
State. Based on health system analysis methodologies proposed by the WHO and the World
Bank, the weaknesses, strengths and difficulties of the health system are evidenced with
respect to the statutory law approved in February 2015. These include the fragmentation and
specialization of services, barriers to access, incentives not aligned with quality, weak
governance, multiple actors with little coordination and an information system that does not
measure results. It is necessary a social agreement, a balance and control of the tension on the
