Dear Sir/ Madam,

Re: Mr. Alfred Billy, aged 52

I am writing to refer Mr. Billy into your care who requires follow up care from your service at home. He
has undergone a surgery as a part of his cancer treatment and is being discharged today.

Mr. Billy was admitted to our hospital with the diagnosis of skin cancer. More specifically, basal cell
carcinoma on his neck. A skin biopsy was performed for pathological study. In addition to this, he was
treated with panadein forte 500 milligram for pain relief.

Prior to this hospitalization, Mr. Billy had always been in good health and has no significant medical
history. This is the first time he has ever been admitted to the hospital.

Mr. Billy is a married man, and lives with his wife, Mrs. Maria, who needs to be contacted in case of an
emergency. He is an alcoholic for the past 30 years as well as a smoker.

Mr. Billy is well at the time of discharge, and does not report any sort of pain. However, he will require
daily observation and the medication needs to be continued for one more week.

It would be greatly appreciated if you could do the needful for him.
