QUITCLAIM AND RELEASE

COME NOW, the undersigned complainant in the above-entitled case before this Office
respectfully manifests:

That for and in consideration of the total sum of TWO HUNDRED FORTY THOUSAND and
FIFTY NINE PESOS (Php 240,059.00) paid to me by the respondents Delsan Office System
Corporation and Odilon A. Santos in settlement of my claims (i.e., reinstatement/separation pay,
backwages, 13th month pay, service incentive leave, regular and special holiday pays, overtime
pay, unpaid commissions, moral and exemplary damages, atty’s fees etc.,) as employee receipt of
which is hereby acknowledged to be my complete and full satisfaction, I hereby release and
discharge said respondents, its owners, officers and stockholders from my claim by way of FULL
SETTLEMENT, due me in connection with my employment with said corporation and its owners,
officers and stockholders.

WITH PREJUDICE

COMES NOW, complainant unto the Office of the Honorable Labor Arbiter most
respectfully states:

That for and in consideration of the total sum of TWO HUNDRED FORTY THOUSAND and
FIFTY NINE PESOS (Php 240,059.00) paid to me by the respondents Delsan Office System
Corporation and Odilon A. Santos in settlement of my claims (i.e., reinstatement/separation pay,
backwages, 13th month pay, service incentive leave, regular and special holiday pays, overtime
pay, unpaid commissions, moral and exemplary damages, atty’s fees etc.,) as employee receipt of
which is hereby acknowledged to be my complete and full satisfaction, I hereby release and

SETTLEMENT, due me in connection with my employment with said corporation and its owners,
officers and stockholders, and as a consequence I HEREBY MOVE TO WITHDRAW/DISMISS
