JOINT AFFIDAVIT OF
CONSENT FOR ADOPTION

We, Benjamin and Veronadia Mendevil, both Filipinos, of legal age, married and
presently residing at____________, Philippines, having been duly sworn in accordance
with law, hereby depose and state that:

1. We are the biological parents of Nigel M. Mendevil who was born on

August 29, 2011 in ______________________. A copy of his Birth Certificate is hereto
attached and made an integral part hereof as Annex “A”.

2 We already have eight (8) children in addition to our son Nigel.

3. Both of us are not gainfully employed and we merely rely on the

generosity of other people for our family’s daily needs.

4. Considering our present situation, we are unable to support the said child
and we believe that it would be for his own interest and benefit that he be given up for
adoption to a set of adoptive parents who are financially capable and emotionally
qualified to raise, rear and support the said child.

5. In view of the foregoing, we hereby signify our written consent, freely,

willingly and voluntarily to have our son Nigel be legally adopted by Spouses
_________ and ___________ in accordance with law.

6. We are ready and willing, freely and voluntarily to transfer our parental
authority over said child in favor of the Spouses ____________ and _____________. We
hereby grant full custody and parental authority to Spouses _________ and _________,
for them to nurture and raise the said child as their own, and to enroll the said child in a
reputable school for the full development of the said child.

7. We are freely giving our full consent for the said child to travel with
Spouses ____________ and _____________ to any country and that we give full
authority to Spouses __________ and __________ to take care of the said child and
watch over his welfare and health during the said trip.

8. We are fully entrusting the health, safety and welfare of the said child to
Spouses ____________ and ___________. In case of a medical emergency, treatment
and/or hospital care, we hereby give our full consent to Spouses __________ and
___________ to act as the parents of the said child.

9. We fully understand the contents of this Joint Affidavit and we have full
knowledge of its consequences. We did not receive any consideration or payment in
exchange for the said child. We have not been forced, intimidated nor threatened to
sign this Affidavit.

10. We are executing this affidavit to attest to the truth of all the foregoing
statements and in support of a prospective Application/Petition for Adoption.
