1) You must file a PA-40 (state) income tax form if you worked and earned income during the previous
calendar year.
2) Print all information on your PA-40 form in CAPITAL LETTERS in BLACK ink.
3) When making entries on your form, ROUND OFF CENTS TO THE NEAREST WHOLE DOLLAR.
4) In the block entitled “Residency Status”, fill in the oval indicating “N Nonresident”.
5) In the block entitled “School Code”, enter 99999 in the code box.
6) When entering your “Gross Compensation” (Line 1A of the form), record the amount listed on your W-2 form
(box 16-state wages, tips, etc.).
7) When entering your “Total PA Tax Withheld” (Line 13 of the form), record the amount listed in Box 17 (state
income tax) on your W-2 form.
8) If payment is due, enclose a check or money order payable to PA Dept. of Revenue. Do not send cash.
Write the last four digits of your SSN, “2012 PA-V” and daytime telephone number on your check.
10) Be sure to sign and date your tax return form. Also, make PHOTOCOPIES of your return and keep the
copies for at least ten years.
11) For mailing purposes, choose the appropriate address listed below. The DEADLINE FOR MAILING is April
15.
