regarding stem cells and umbilical cord blood banking among post natal mother in
selected hospital of Jabalpur.
Redefining health in its endeavor to bring a leading edge leading concepts is proud to
introduce stem cells therapy. Umbilical cord stem cells therapy is utilized through the
blood of umbilical cord after the placenta has been expelled from the body after the
baby is born. As a rich source of stem cells many parents today are ‘banking’ their
children’s umbilical cord cells in they are needed for curing disease in the future.
Objective

• To assess the pretest knowledge regarding stem cell and umbilical cord blood
banking knowledge among post natal mother in selected hospital of Jabalpur.

• To assess the posttest knowledge regarding stem cell and umbilical cord blood
banking among post natal mother in selected hospital of Jabalpur.

• To compare pretest and post test knowledge regarding stem cell and umbilical
cord blood banking among post natal mother in selected hospital in Jabalpur.

Hypothesis

H1 = There will be significant difference between then mean pretest score and mean
post test score regarding stem cell and umbilical cord blood banking knowledge
among the post natal mother.

H0 = There will be no significant difference between the mean pretest scare and mean
post test scar regarding stem cell and umbilical cord blood banding knowledge
among the post natal mother.

Hence the investigator of the opinion to improve the knowledge regarding stem cells
umbilical cord blood banking.
The present study is a pre-experimental evaluation approach to evaluative the
effectiveness of plan teaching program on knowledge regarding stem cells and
umbilical cord blood banking among post natal mother in selected hospital of Jabalpur
(M.P.). The study was conducted in an Lady Elgin Hospital of Jabalpur (M.P.). The
sample size consists of 30 post natal mother. Purposive sampling techniques was use to
select the sample respondent.
The data collection was done by structured questionnaire. The tool was tested for
validity, reliability was found to be 0.94 and item analysis was carryout feasible was
obtaining through pilot study.

Majority of 26(86.66%) had poor knowledge and 4 (13.33%) had average knowledge
the average mean of total pretest knowledge score is 5.03. Majority of 16(53.33%) had
a poor knowledge, 13(43.33%) had average knowledge and 1(3.33%) had a good
knowledge after administration of structured teaching program and the average mean of
total post test knowledge score is 11.06. The finding concluded that structured teaching
program was effective and improving knowledge regarding stem cells and umbilical
cord blood banking among post natal mothers.
INTRODUCTION
CHAPTER 1
INTRODUCTION
"Stem cell research must be carried out in an ethical manner in a way that
respects the sancity of human life."

John Bochrer

Redefining health in its endeavour to bring a leading edge healing concepts is proud to
introduce stem cell therapy. Stem cell therapy. Stem cells therapy is the most advanced
technology available globally to repair the bodys failing system. Stem cells have the
remarkable potential to develop into may different cell types in the body.

Umbilical cord stem cells therapy is utilized through the blood of umbilical cord. After
the placenta has been expelled from the body after a baby in born. As a rich source of
stem cells many parents today are 'banking' their children's umbilical cord cells in care
they are needed for using disease in the future.

The definitive statistics on the rate of stem cell transplantation in the us were published
in a child up to 20 years age receiving an autologus stem cells transplant to be 1 in 5000
and increasing to 1 in 450 by age 70 (Nietfeld et al. 2008)

A study was conducted to assess the effectiveness of self instructional module on the
knowledge regarding placental cord blood banking among ante natal mother in selected
hospitals in Kasargoad, by approaching one group pre test post test design by
conveying sampling. The mean post test knowledge (x2=43.17) was higher than the
mean pre test knowledge (x1=30.40). The study concluded that there was no association
between the pretest knowledge scores and the selected demographic variables.

"Dinc & Sachin (2009)"

A prospective study was performed with a standardized anonymous questionnaire

distributed to 131 swiss women 6 month after cord blood donation to assess attitude of
swiss mother towards unrelated umbilical cord blood banking. The vast majority
(90.1%) stated that they would donate umbilical cord blood again a significant
correlation (p=0.01) was found between repative attitude and the decision net to donate
umblicial cord blood again. Additionally it was observed that woman who had a
negative experience concerning the donation of cord blood would net donate again
(p=0.004). This study reveals a high degree of satisfaction of unrelated umbilical cord
blood donation for banking is woman 6 months after delivery.

"Enrico Danzer, wolfgang Holzgre Swiss (2000)"

Need of the Study

A cord blood bank is a facility which store umbilical cord blood for future use.
Research have found scarcity of knowledge regarding umbilical cord blood banking
among post natal mother therefore study was conducted to evaluate the effectiveness of
structured teaching programme regarding stemcell and umbilical cord blood banking.

Stem cells have the ability to build energy tissue in the human body hence they have
great potential for future therapeutic uses in tissue regeneration and repair.

A large number of stem cells are normally found in the blood of newborn babies. After
birth the blood that is left behind in the placenta and umbilical cord (known as cord
blood) can be taken and stored for later use in a stem cell transplantation.

During the clinical postings the investigators felt the there is an inadequate awareness
regarding stem cell banking among mothers. So the investigators interest and felt a
need to conduct this study.

PROBLEM STATEMENT

A study to assess the effectiveness of structured teaching programme on knowledge

regarding stem cell and umbilical cord blood banking among post natal mothers in
selected hospital of Jabalpur.

knowledge of mother about stem cells umbilical cord blood banking which can be
measured by post test.

Structured teaching programme

Systematically developed instruction method and teaching and design for post natal
mother providing knowledge about stemcell and umbilical cord blood banking.

Stem cells

Stem cells are class of undifferentiated cell that are able to differentiate into specified
cell types commonly stem.

Post natal mother

It refers to the mother after delivery to whom structured teaching programme is given.

Cord blood banking

It is popular trend among new and expected parents but it worth the cost and potential
future.

Limitation

• The sample size limited to 30.

• The sample limited to postnatal women who can understand hindi language.

• The study is limited to mothers of Elgin hospital.

Conceptual frame work

Conceptual frame work represents a less formal attempt of organizing phenomina, it

deals with abstraction or concepts that are assembled by the virtue to their relevant to a
common then the conceptual framework can serve as spring board for the generation of
research hypothesis and can provide an important contest for the further research (Polit
& hungler 1990)

The conceptual frame work is a theoretical approach to the study of problem. That is
scientifically based and which lay emphasis on solution arrangement and clarification
of concepts. The conceptual framework formalize the thinking process so that other
may read and know the basics to the research problem.

According to Polit and Hungler, 1999 a conceptual framework is a cohesive support

linking of selected interrelated objects observation, event and experience. It serves as a
guideline to research and springboard for the generation of research hypothesis.

The conceptual framework selected for the study is based on Imogene on Kings "goal
attainment model".

The theory focus on interpersonal system reflects kings belief that the practice of
nursing on differentiated from that of other health professions by what nurses do with
and for individual. The major elements of theory of goal attainment are seen in the
interpersonal system in which two people who are usually strangers, come together in a
health care organization to help and be helped to maintain a state and health that
permits functioning in roles.

The concept of the theory is perception, action, interaction transaction. The concepts
are interrelated in every nursing situation. These forms are defined as concepts in the
conceptual framework.

Perception

Perception is each person's representation of reality the element of perception are

importing of energy from the environment and organizing it by information in the
forms of overt behaviours. In this study investigator perceives, learning needs of the
post natal mother. Mother perceives lack of knowledge to stem cells and umbilical cord
blood banking.

Action

Action refers to activity to achive the goal what the individual perceives. In the study it
is mutual goal setting to knowledge regarding stem cells and umbilical cord blood
banking. Investigator prepares structured questionnaire to assess the knowledge of
postnatal mother regarding stem cell and umbilical cord blood banking and prepare
PTP.

Interaction

Interaction refers to the perception an conception between or person and environment

or between two or more person in this study, investigator administer the structured
questionnaire to assess the knowledge and administering planned teaching on stem cells
and umbilical cord blood banking. Mother responds to structure questionnaire and
listen to the planned teaching programme.

Transaction

It is process of interaction in which human beings communicate with the environment

to achieve goals that are valued and directs human behaviour. In this study there is gain
in knowledge regarding stem cells and umbilical cord blood banking.

REVIEW OF LITERATURE

This chapter presents a review of literature related to the research topic. It involves
systematic identification of a research problem and development or refinement of research
questions or hypothesis, orientation to what is known and not known about an area of injury.

Review of the literature is a key step in research process. The major goal of the review of
literature is develop a strong knowledge base to carryout research and other non research
scholarly activities is educational area such knowledge enhance the writing of scholarity
paper by students and faculty.

Review of literature has been done under following headings:-

1. Literature related to stem cells and umbilical cord blood banking.
2. Literature related to knowledge of women regarding stem cells.
3. Literature related to effectiveness of structured teaching program.

1. literature related to stem cells and umbilical cord banking:-

A study was conducted on assessment of feasibility of related cord blood collecting the
appropriateness of storage and the final suitability for transplantation. 63 families were
enrolled in this study. A total of 72 cord blood unit were collected and stores for 64 patients.
HLA typing was carried out for 34 donor- recipient couples and most unit (72%) mismatched
with the related patients. About 60% of collection had acceptable for transplantation. The
results shows that banking of related cord blood units is an important resources then, this
public service should be supported and enhanced.
- Larolyn Troeger, Ulrike kostka , 2003

A study was conducted in USA to overcome the barriers to umbilical cord blood
transplantation. The study results showed that there has been marked increase in the
utilization of umbilical cord blood transplantation (UCBT) in recent year, especially in the
pediatric population, however graft failure, delayed engraftment, and profound delay in
immune reconstitution led to significant morbidity and transplant across human Leucocytic
Antigen (HLA) disparities, rapid procurement time and decreased graft versus host disease
(GVHD) seen with unit cord blood transplant made it a promising stem cell source and while
barriers visited, consistent program was being made to overcome them.
- “Fernamdez(2011)”

An experimental study was conducted to assess the effectiveness of human fetal critical brain
tissue on 12 patients with parkinsonism aged 45-75 years, who were not responding for anti
parkinsonism drug Sampling is purposively done. Evaluation of the patient after one month
revealed, mild improvement up to 33.3% in 41.6% of cares and moderate improvement (up to
66.6%) in mother 41.6% of cares. The study concluded the foctal cortical brain tissue can be
used for the treatment of parkinsonism.
- “Stephen Ski Hung Suen(2011)”

2. literature related to knowledge of women regarding stem cells:-

An exploratory descriptive study was conducted in 2 antenatal outpatient clinics is Istanbul.
The sample consisted of 334 pregnant women now during routine prenatal exists. Data were
collected in interview by using an interview form developed by the researchers according to
the literature. The form included demographics characteristics of participants and 20
questions about stem cells, storing cord blood and banking and 10 indqendent attitude
statements. The result revealed that majority of the participants had a lack of knowledge
about stem cells and cord blood banking and wanted more information.
- H. Dine and N. H. Sahin, 2009

A prospective study was performed with a standardized anonymous questionnaire distributed

to 131 Swiss women 6 months of after CB donation to assess attitude of Swiss mothers
towards unrelated umbilical cord blood banking by random sampling method. The vast
majority (90.1%) stated that they would donate umbilical CB again, a significant correlation
(p=0.01) was found between negative attitude and the decision not to donate umbilical CB
again. Additionally concerning the donation of CB would not donate again (p=0.004). This
study reveals a high degree of satisfaction of unrelated umbilical CB donation for bank in
women 6 months after delivery.

Swiss
A study was conducted to determine patients knowledge of umbilical cord blood banking
(UCBB) among 425 patients by conveniency sampling, a questionnaire was administered is
part 1 and 2, 37% had we knowledge of UCBB. Older patient and those with higher degree of
education were more aware of UCBB and the greatest disparity of knowledge was noted
among native American patients (p<0.001) patients indicating familiarity with UCBB, 2-6%
felt “extremely knowledge “ , while 74% felt “minimally informed”. 50 % of the patients
were misinformed about UCBB 7% of patient were not planning UCBB. Only 14% of
patients were educated about UCBB. The result conducted that patients are poorly informed
about UCBB, especially ethnic minorities, younger patients and those with lesser degree of
education.
- Jorden H. Perlow, M.D., USA, 2004

An exploratory descriptive research study conducted on knowledge and attitude of pregnant

women with regard to collection, testing and banking cord blood stem cells in north America
at Varivillas hospital in Bangalore among 443 antenatal mothers. A non probability purposive
sampling technique was used to select the samples. In this study, most of the antenatal mother
had poor knowledge score (95%) regarding collection and storage of cord blood banking and
(5%) had no knowledge. The results conclude that there is poor knowledge level among
antenatal mothers.
- M.S. Vijaylaksmi (2013)

A prospective study was performed to evaluate previous knowledge of the possible medical
use of cord blood and acceptance of cord blood banking among pregnant women at the
university of board women’s Hospital pregnant outpatient clinic by using a structured
anonymous questionnaire from 300 questionnaire, 250(83%) were returned, and 245 could be
evaluated for final analysis. Only 40% indicated low knowledge about cord blood banking
(95%) supported the idea of UCBB 93% stated that they would age to donate the cord blood.
This study conclude with high acceptance of umbilical cord blood donation for banking and
stem cells transplantation.
- Surbek DV Isiebe A, 1998

3. Literature related to effectiveness of structured teaching program:-

An evaluator study was carried out in selected nursing college at Mangalore to evaluate the
effectiveness of structured teaching programme on stem cell. The sample comprised for 60
nursing students by purposive sampling technique. The mean percentage knowledge score of
pre-test was maximum in the area of knowledge on therapy (51.1%) and minimum in area of
type (28.8%) where as the mean percentage knowledge score of post test was maximum in
the area of therapy (90.1%) and less in the area of anatomy(70%). It was concluded the
structured teaching programme is an effective teaching strategy in improving the knowledge
of nursing students on stem cell therapy.
- Sigiann Varghere, Mangalore, 2013

A descriptive study was conducted on assessment of knowledge on cord blood banking

among 127 antenatal mothers in hospital Kluang, Johor, Malasiya. Data collected through a
set of questionnaire. In this study it was found out that 100% of antenatal mothers have little
knowledge towards cord blood banking. 18.2% of them are aware about cord blood banking
and 91.8% were not. The finding of the study concluded that majority of antenatal mothers
were having no knowledge about cord blood banking.

is accurate & Interpretable. The research design indicate whether there an investigation and
what they made the methods to being to controlled extrapraneous variable and enhance the
study interpretability. the training and frequency of data collection , the setting the nature of
communication with the subject, the selection of research design depend upon the purpose of
the study, research approach a variable to be studied.
The research design for present study in pre-experimental one group pretest post test design
to measure the effectiveness of the STP for post natal mother.
An experimental design is the blue print of the proceeding that enable the researcher to test
the hypothesis by reaching valid conclusion about the relationship between independent and
dependent variable.
In this design introduce above measure before and after the planned expose, which depicted
as 01 and 02 respectively. in the present study the base means was knowledge test, the
experiment variable was STP.
Thus the research design can be present as:-
