AFFIDAVIT OF LOSS

I, LITO A. PERMEJO, Filipino, of legal age, single, and a resident of Pinagsabangan II, Naujan,
Oriental Mindoro, after being duly sworn in accordance with law, depose and state:

1. That I am the authorized representative of JOVEN GAAS SAGUID who is the only surviving
heir of FLORENCIO GAAS and ANTONINA GAAS pursuant to a Special Power of Attorney
executed before Notary Public Atty. Vic Ruskin M. Ong dated May 13, 2014 under Doc. No. 107;
Page No. 23; Book No. II; Series of 2014.

2. That he is the legitimate owner of a parcel of land which is more particularly described as
follows:

A parcel of land (Lot 634 Pls-83, (Philcusa-Foa), Pinamalayan Public Land

Subdivision, L.R.C. Record No. ), situated in the Municipality of Pinamalayan,
Province of Or. Mindoro. Bounded on the SE., and SW., along lines 1-2-3-4 by
Papandungin River, on the NW., along lines 4-5-6 by Lot 631. (Teodulo Mantaring); on
the E., along lines 6-7-1 by Lot 627, Gaudencio Mantaring.

Beginning at a point marked “1” on plan, being S. 3 deg. 31’E., 8138.38 m. from
BLLM No. 1, Pls-83, Pinamalayan Public Land Subdivision.
Thence:

S. 57 deg. 15’W., 53.55 m. to point 2;

N. 40 deg . 49’W., 18.37m. to point 3;
N. 30 deg. 41’W., 114.62m. to point 4;
N. 73 deg. 37”E., 104.03m. to point 5;
N. 89 deg. 53’E., 14.12m. to point 6;
S. 2 deg. 10’E., 36.93m. to point 7;
S. 0deg. 10’E., 75.98m. to the point of beginning;

Containing an area of TEN THOUSAND FOUR HUNDRED FORTY EIGHT

(10,448), Square Meters. All points referred to are indicated on the plan and are marked
on the ground by P.S. Cyl. Conc. Mons. 15x60 cm.; Bearings True:
date of original survey, and was approved on November 2,1962.

3. That sometime on the year 1962, the ORIGINAL COPY OF TRANSFER CERTIFICATE OF
TITLE was lost.

4. That recently the owner tried to look for the said TRANSFER CERTIFICATE OF TITLE but the
same could not be found.

5. That to my knowledge, there is no deed or other instrument affecting the property which had been
presented for registration during the loss of the same document.

6. That despite diligent search and efforts, the same could no longer be found such that I now
believe that it is now lost beyond recovery.

7. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support
the application for the issuance of new OWNER’S DUPLICATE COPY OF TRANSFER
CERTIFICATE OF TITLE by the Office of the Register of Deeds of the City of Calapan in lieu of
the one which was lost.
 IN WITNESS WHEREOF, I have hereunto set my hand this 24 th day of June 2014 at Gloria, Oriental
Mindoro, Philippines.
