Candidate Profile Questionnaire


To better understand you both professionally and personally, and in order to present the best opportunities, the
following is a list of questions addressing topics relevant to those goals. Some questions are of a personal nature and
may be uncomfortable for some candidates to answer but keep in mind that our clients need this information, which is
why we ask. Please let us know if you have a problem answering any of these questions. You can provide the answer
to each question directly beneath it.

(DO NOT TAKE MORE THAN 15-20 MINUTES TO FILL OUT THIS QUESTIONNAIRE. SHORT,
SUCCINCT RESPONSES ARE WHAT WE ARE LOOKING FOR.)
1. Please Provide the Following Contact Information: Your Name, Current Home Address, Home and Cell Numbers,
Personal Email Address, and Today’s Date.

2. Please summarize your qualifications (focus on the last 3-5 years including brief organization overview, role,
reporting structure, and reason for leaving last job).

3. What is your current citizenship/residency status? Are you legally authorized to work in the US for any employer
without restrictions?

4. What type of an opportunity are you currently looking for? A few items to consider would be the general title
(auditor, controller, financial analyst, etc.), type of position (private industry or public accounting), industry
(manufacturing, non-profit, etc.), environment (size and culture of an organization), and location (city and state).

5. Are there any specific companies you would like to work for? Are there any you would prefer not to work for?
Are there any clients we should be aware of that might create a conflict of interest?

6. On a scale from 1-10 (10 being the highest), how serious are you about accepting a new position, assuming it meets
most of your requirements, and when would you be able to start?

7. Why are you looking to make a change?

8. What have you enjoyed most about your present job responsibilities?

9. What have you enjoyed least about your present job responsibilities?

10. In your previous or current position, what do you consider to be your greatest accomplishment(s)?

11. What type of management style and culture brings out your best performance?

12. If you are applying for a specific position, what about this opportunity excited you or drew your interest the most?

13. What are your expectations in a new position in regards to career fulfillment/desired career path?

14. Do you have any non-compete agreements that would prevent you from considering any opportunities?
15. Are there any financial obligations (i.e., school loans, pre-tax cash, etc.) or other monies that you would owe or lose
out on should you pursue a new opportunity?

16. Are you vested, and if so, would you be losing any money if you started a new job?

17. Are you working with other recruiters and have you interviewed or spoken to any firms/companies? If yes, please
list companies/recruiters you have spoken to and/or interviewed with and whether you have any pending offers.

18. Are you planning a vacation, training, or significant time off (more than two weeks) in the next 6 months?

19. How many hours are you working on a weekly basis (busy season vs. non-busy season)?

20. Are you pursuing any degrees or professional certificates (i.e., MBA, MST, CPA, CIA, CISA, and/or graduate
programs) that are not on your resume? If you are in the process of taking an exam for one of these, how many
parts have you passed?

21. What area of your skills are you planning to improve upon in the next year? Why?

22. If you have a CPA license, is it in escrow or not active?

23. Are you open to positions in other states besides Michigan? If yes, please list the cities/states that interest you.

24. List the top 5 concerns you have for your next position (dollars, commute, opportunity, etc).

25. Please briefly describe your personality. What attributes, both positive and negative, best describe you?

26. Where do you see yourself within the next 3-7 years?

27. Have you ever been convicted of a felony, etc.?

28. How did you hear about our organization (Monster posting, referral, etc.)?

29. Personal background (OPTIONAL): Where are you from originally? Are you married? Do you have any kids?
What are some of your hobbies? Etc.

30. Please include 4 references, preferably 2 peers and 2 supervisors, along with their email addresses. We utilize a
standard reference form that we email out. If possible, let these individuals know we may be in contact with them.
