Public Debt that has been assigned to me and my associated Trusts. My Live Birth was __________
with the state of Iowa registration performed on ____________, under the "Certificate of Live
Birth" number ___-__-_______.
This letter is to inform you that the Living Individual, Your : Name as the Fiduciary of the
Your Name “UBO - Cestui Que” Trust, is Rejecting of All Benefits from both the Social Security
Trust and the Birth Trust. I have two certified copies of my “Certificate of Live Birth”; and my
“Social Security Card” and will sign them over upon demand from you. These documents would
be known to the Trusts as “INDIVIDUAL CERTIFICATE OF INTEREST” or better known to a
trust as the Title of Equitability Certificate.
If there are any additional unaddressed requirement(s) or forms required, please submit them
to or contact, the Living Individual Your : Name, at the above address, within three (3) working
DAYS of the date stamp of this letters receipt and I will respond immediately.
If this is not the correct Department and/or other Departments are needed to be contacted,
please either forward it to them or contact them and notify me forthwith.
