4. That I have been residing at of H.No 217, Street 5, Korang Town, Islamabad since 01 years
and want to open a clinic in the field of dentistry as mentioned in the application submitted at
Punjab Health Care Commission.
5. That I am will deposit installment time to time, that I shall declare indemnity that I shall
deposit my loan in time.
