Care Coordination and Navigation
• Responsible for all coordination of mental health care for clinic patients and ensure the effective management of all
incoming mental health referrals.
• Work closely with our mental health and primary care providers to efficiently coordinate patient care.
• Able to develop a strong and consistent working relationship with other staff members.
• Liaising with patients and healthcare providers on a daily basis.

• Experience with the homeless and criminal justice population, as well as individuals with substance abuse and mental

Patient Advocacy
• Advocate and point of contact for patients in assisting with navigating medical systems and available resources.

Francisco, or other health coverage programs as necessary.
• Provide education and support to current clients and interested community members about the Affordable Care Act
and their health coverage options to increase MediCal enrollment and to support linkages to and retention in health
care services.
• Identify barriers to medical treatment, ensure access to appropriate resources, improve patient experience and resolve
issues that could impact smooth care progression.
• Effectively assist patients in problem solving potential issues related to the health care system, financial or social
