BELL’S PALSY:

I’ll wash my hand.

Good Day. I’m Dr._________. I’m the HMO in this hospital. How do you want me to address you?
Today I’m gonna do a physical examination of face, I’m gonna have a look, feel and I’ll ask you to do
some movements. Is that alright with you?

Before I start. Are you in pain? Where exactly is the pain? Do you need any painkillers? During the
examination if I hurt you or you feel uncomfortable, just let me know so that I could stop.

 GA
 VS
 FACE: look, CN7 (4)
 Eye: look: ulcer, reflex: corneal 5 in 7 out
 Ear: look: herpes, hearing
 Parotid gland: look, feel, tender, regular or irregular

 GA:
1. I would like to see the general appearance of my patient?
2. The patient has a left sided facial droop.
3. On eye inspection, there were no ulcers of the cornea.
4. On ear inspection, there were no vesicles, ulcers or rashes that may indicate a herpes
infection.
 VS: I will check for the vital signs including the blood pressure, heart rate, respiratory rate,
temperature and O2 saturation.
 FACE: look, CN7 (4) I will examine the cranial nerve 7 or facial nerve.

1. Did you notice any changes in your sense of taste lately?

2. Can you please look up and wrinkle your forehead?
3. Please close your eyes tightly and don’t let me open it?
4. Can you please puff out your cheeks with your mouth closed?
5. Can you please show your teeth?
The nasolabial folds are normal.
 EYE: On eye inspection, there were no ulcers of the cornea.
 Look: ulcer, reflex: corneal 5 in 7 out
 EAR
 Look: herpes, hearing
 On ear inspection, there were no vesicles, ulcers or rashes that may indicate a herpes
infection.
 PAROTID GLAND:
 Look, Feel, Tenderness, Regular or irregular
 I would like to examine if there is any enlargement of the parotid gland. Can you please open
your mouth? There was no dental problem or ulcers on the tongue.
 I would like to palpate the salivary gland to check for any discharge or salivary stone. On
palpation, there was no tenderness elicited.
 The boarder of the parotid gland is normal.

To complete my examination, I would like to examine the lymph nodes for any enlargement and do a
complete examination of the cranial nerves and the upper and lower limb neurological examination.
