Journal Club Research Article Critique Form Reviewer name: Tammy Tarner Unit: Med Surg Date: 03/06/2018_________ Approved 3/7/18: Research study:_Hardwiring Bedside Shift Report (Scheidenhelm, Reitz) https://insights.ovid.com/pubmed?pmid=28157819 1. Description of the study • The purpose of the research is: To increase nurse compliance with bedside report and increase patient satisfaction scores. • Explain why this problem is significant to nursing practice: Bedside report is an evidence based practice that is used to increase patient involvement in care and improve patient satisfaction, both Joint Commission requirements. 2. Evaluation of literature • Describe the previous research pertaining to the topic that the author’s reference (hint: look for a literature review section in the article) Lit review found that bedside report allows patients to be actively involved in their care. A standardized approach to bedside report and manager support of nurses can enhance shift handoff and improve patient safety, outcomes, patient and nurse satisfaction, time management, and accountability. 3. Study sample The study sample was obtained from: (hint: describe the population and where the study is performed) A 46-bed medical/surgical unit and a 12-bed obstetrics unit within a 149-bed community hospital • What is the sample size? Data was obtained from Press Ganey results for 4 months after implementation of bedside report. Results were obtained from 190 medical/surgical patients and 99 OB patients • List the inclusion criteria used in the study. All patients were included in study • List the exclusion criteria used in the study. There were no exclusions 4. Study methods/design • Describe or identify the study design (hint: quantitative/qualitative, experimental, meta-analysis, etc). A quasi-experimental, between-group, preimplementation and postimplementation comparison of patient satisfaction scores from returned surveys was used. • Describe the study procedures. (hint: describe the intervention and how the data was collected). Nurses were trained on bedside shift reporting, addressing barriers, using a standardized approach, and evidence supporting the process. Nurses were given a tool kit with detailed process information. A trained team monitored bedside report using a competency checklist for 2 months following implementation, then again at 1 and 3 months post implementation. Press Ganey scores for 3 statements, “nurses kept you informed”, “staff included you in decisions regarding treatment”, and “nurses explained in a way you understand”, were compared preimplementation and 4 months postimplementation. 5. Results • Describe the results of the study. Nurse compliance with bedside report increased on both units. Patient satisfaction mean scores, percentile rank, and percentage of “always” responses improved on all statesments for the medical/surgical floor. For OB, the satisfaction mean score improved on 1 statement and decreased slightly on the other statements, but the percentile rank for OB remained high at 99. Whereas med/surg demonstrated high improvement, the OB unit saw less improvement, possibly because the OB unit had a higher percentage of compliance with bedside reporting prior to the study. 6. Clinical significance • Explain how you will use this information for your nursing practice at SJMC. Information from this study can be used to assist in the implementation of bedside reporting on the medical surgical unit at St John Owasso. Methods used to increase nurse compliance at the sample hospital are applicable to St John Owasso.
