I, ALVIN CHRISTOPHER FERNANDO DELA CRUZ, of legal age,

Filipino,single and with present residence and postal address at Blk 5 Lot 15
Cetrin Street, Brgy. Meminje, General Mariano Alvarez City after being duly
sworn to in accordance with law, hereby depose and state:

That sometime last November 06, 2013, my Driver’s License was confiscated
by the Land Transportation Office while I was driving a motorcycle;

That after the incident, the motorcycle was repossessed by the financing
institution;

That such motorcycle was not registered under my name as I just borrowed
it during that time;

That I am executing this affidavit in good faith and without any intention to
defraud or deceive;

As such, I am executing this Affidavit to attest to the truth of the foregoing

specifically to redeem the Driver’s License and for other reliefs as may be
just and equitable.
