STREPTOKINASE INFUSION
Preparation :
1.5 megaunit / 2 mls

Dilute 1.5 mega unit of Streptokinase (2mls) with 8 mls of Water for injection
10 mls dilution

Put the 10 mls dilution into 90 mls of Dextrose 5% or Normal Saline in a


Microchamber

Run over 1 hour

Preparation :
100 iu / ml

Use 50 mls Syringe


Take 50 iu of insulin (0.5 mls )
Dilute with 2 mls Haemacel / Gelafundin + 48 mls of Normal Saline = 50 mls dilution

Rate :
1 ml / hour = 1 iu /hour

Preparation :
150 mg / 3 mls

Loading Dose :
Dilute 2 ampoules of Amiodarone ( 300 mg / 6 mls )
in 100 mls of Dextrose 5%
use microchamber
run over 1 hour

Maintenance Dose :
Dilute 6 ampoules of Amiodarone (900 mg/18 mls )
in 500mls of Dextrose 5%
run over 24 hours ~ 20 mls/hour

N-ACETYL CYSTEIN INFUSION

Preparation :
2 gm / 10 mls

Regime (dosage) :
150mg/kg in 200 mls of Dextrose 5% over 15 min

then 50mg/kg in 500 mls of Dextrose 5% over 4 hours

then 100 mg/kg in 1 litre of Dextrose over 16 hours

e.g : Body weight = 70 kg


150mg/kg = 150 x 70 = 10500mg =10.5gm
1 ampoule = 2 gm
10.5 gm = 10.5 = 5.25 ampoule ~ 5 ampoules

5 ampoules of N- AcetylCystein in 200 mls of Dextrose 5% over 15 min.

AMINOPHYLINE INFUSION :

Preparation :
250 mg / 10 mls

Slow Bolus :
125 mg 250 mg ( 6mg/kg) in 50mls Normal Saline
use syringe pump
run over 20- 30 min

Infusion :
Dose : 0.5 0.9 mg /kg/hour

Dilute 250 mg in 50 mls syringe with Normal Saline = 50 mls dilution


1mls = 250 = 5mg

e.g : Body Weight 70 kg


0.5 mg/kg/hour = 0.5 x 70 = 35mg/hour
35mg/hour = 35 = 7 mls/hour

Dilute 5mg of Nitrobid with 50 mls of Normal Saline

LIGNOCAINE ( for stable Ventricular Tarchycardia )


Preparation : Preparation :

100mg in 5 mls ( 20mg in 1 ml) 500mg / 5 mls (100 mg in 1 ml )

1 st Loading dose : Infusion :


1.0 1.5 mg / kg 500mg(5mls) of lidocaine
to give slow bolus in 2 mins add 45 mls of Dextrose 5 %
= 50 mls dilution
2 nd Loading Dose :(if needed ) 50mls = 500mg
0.75 mg 1.5mg /kg 1ml = 10mg
to give 5- 10 minutes after the Ist Loading Dose
to give slow bolus in 2 mins Dose : 1 4 mg / min

Preparation : 2.47 gm (2.5gm) in 5mls ( 0.5 gm in 1ml)

I/V Slow Bolus :


4.0 gm (8mls ) in 10 15 mins
4.0gm (8 mls ) + 12 mls of Dextrose 5 % = 20 mls dilution
use syringe pump and run in 10 15 mins
Rate : 120mls - 80 mls /hour

Infusion :
Dose : 0.5 2.0 gm/hour
2.5 gm (5mls ) + 45 mls of Dextrose 5 % = 50 mls dilution
use syringe pump and run over 5 hours
Rate : 10 mls/hour (0.5 gm/hour)
Continue for 24 hours

Monitor : Antidote : I/V Calcium Gluconate 10mls


over 10 min
Knee reflex off infusion if absent of knee reflex
Respiratory rate of if RR < 16/min

POLYVALENT / MONOVALENT ANTI-SNAKE VENOM SERUM

Dilute 2 vials of anti-snake Venom Serum with 10mls of water for injection (each vial) = 20 mls
dilution .

Put the 20 mls dilution into 30mls Normal Saline = 50 mls dilution

Use syringe pump

Run in 1 - 2 hours

In patient with hx of allergy - asthma,eczema or drug allegry to give :


Antihistamine
Hydrocortisone
15-30 minutes before administration of the anti-snake venom serum

If allergic reaction /anaphylactic reaction develop after the administration of the anti snake venom
serum :
Intramuscular injection of 1 mls of 1 : 1000 adrenaline

PRALIDOXIME for Organophosphate Poisoning

Preparation :
Single dose vial 1 gm

Add 20 mls of sterile water for injection into the vial = 20 mls dilution
Add the 20 mls dilution of Pralidoxime into 80 mls of Normal Saline in
Microchamber drip = 100mls
Run over 15 30 mins
(Rate : 400mls-200mls/hour )

If after 1 hour muscle weakness has not been relieved second dose is
indicated.

Additional doses may be given cautiously if muscke weakness persist.

SODIUM BICARBONATE ( for metabolic Acidosis pH < 7.2 )

Preparation :
Sodium Bicarbonate 8.4 % ( 10 mls)
1 mls contain 1mmol (1 mEq/l ) of Sodium and Bicarbonate ion.

Dose :
Adult : Dose (mmol) = Base Deficit ( mEq/l ) x Body Weight (kg) = x mls of 8.4 % Sodium Bicarbonate

Administer (half of the calculated amount ) over 20 - 30 min in a syringe pump


Monitor the acid-base balance

Paeds : (use the 4.2 % solution )


< 5 kg : Dose (mmol) = Base deficit (mEq/l) x Body Weight (kg) = x mls of 4.2% Sodium
